【摘要】：目的:探討多學科系統(tǒng)化管理兇險性前置胎盤(PPP)對母兒妊娠結局的影響。方法:2013年1月至2015年2月在南京大學醫(yī)學院附屬鼓樓醫(yī)院住院分娩的PPP孕婦41例,其中經(jīng)包括自體血貯備、術前核查、髂總動脈球囊預置等內容在內的多學科系統(tǒng)化管理者為系統(tǒng)管理組20例,未接受妊娠期管理者為對照組21例。分析這些病例的母兒妊娠結局。結果:2013年、2014年和2015年1~2月,兇險性前置胎盤分別占同期分娩總數(shù)的0.5‰(2/4402)、4.3‰(25/5762)和15.5‰(14/905),發(fā)生率大幅迅速增加,差異顯著(P0.001)。系統(tǒng)管理組和對照組的產(chǎn)后出血量分別為710ml(230~6000ml)和1500ml(310~4515ml)ml,出血率分別為30.0%和61.9%。系統(tǒng)管理組的產(chǎn)后出血量和出血率均低于對照組(P=0.06,P=0.07);產(chǎn)前自體血貯備量顯著增高(P≤0.05),異體濃縮紅細胞(RBC)輸注量達到或超過4U以上大量輸血孕婦明顯減少,早期發(fā)病率為15.0%,顯著低于對照組的52.4%(P≤0.05)。結論:多學科系統(tǒng)化管理流程能減少PPP圍術期的嚴重并發(fā)癥。
[Abstract]:Objective: to investigate the effect of multi-disciplinary systematic management of dangerous placenta previa (PPP) on maternal and fetal pregnancy outcome. Methods: from January 2013 to February 2015, 41 pregnant women with PPP who were hospitalized and delivered in Gulou Hospital affiliated to Medical College of Nanjing University, including autologous blood reserve, were examined before operation. The systematic management of the common iliac artery included 20 cases in the system management group and 21 cases in the control group. The maternal and fetal outcomes of these cases were analysed.
