【摘要】：目的全面評價單孔腹腔鏡手術(shù)(LESS)治療輸卵管妊娠的安全性、可行性及其他潛在優(yōu)勢。方法檢索Pubmed、the Cochrane Library、Web of Science等英文及知網(wǎng)、中國生物醫(yī)學(xué)文獻(xiàn)等中文數(shù)據(jù)庫中有關(guān)單孔與傳統(tǒng)腹腔鏡下輸卵管妊娠手術(shù)治療的對比研究,對選用的研究進(jìn)行質(zhì)量評價,運(yùn)用Rev Man5.3軟件行數(shù)據(jù)合并分析。結(jié)果最終納入2個隨機(jī)對照研究(RCT)和14個病例-對照研究,共1 541個患者,Meta分析結(jié)果提示,單孔對比多孔腹腔鏡手術(shù),延長了手術(shù)時間[加權(quán)均數(shù)差(WMD)=8.54,95%CI(2.43,14.64),P=0.006],但并沒有增加總并發(fā)癥[比值比(OR)=0.68,95%CI(0.27,1.71),P=0.410]及術(shù)中失血量[WMD=-0.01,95%CI(-2.51,2.48),P=0.990],也未延長術(shù)后腸排氣時間[WMD=-0.45,95%CI(-1.72,0.82),P=0.490],且能縮短術(shù)后住院時間[WMD=-0.40,95%CI(-0.75,-0.06),P=0.020],減少術(shù)后止痛治療[OR^=0.38,95%CI(0.22,0.67),P=0.000]。結(jié)論 LESS治療輸卵管妊娠,是安全、可行的,而且有住院時間短,術(shù)后疼痛輕的優(yōu)點(diǎn),有望取代傳統(tǒng)腹腔鏡手術(shù)(CLS)。
[Abstract]:Objective to evaluate the safety, feasibility and other potential advantages of single-hole laparoscopic surgery for tubal pregnancy. Methods A comparative study on single hole and traditional laparoscopic tubal pregnancy surgery was conducted in Pubmedus Cochrane Library of Science, Chinese biomedical literature and other Chinese databases, and the quality of the selected studies was evaluated.
