【摘要】：目的探討早孕期規(guī)范化超聲檢查在胎兒畸形篩查中的應(yīng)用價值。方法選取行11~13+6周早孕期篩查的孕婦2 670例,采用常規(guī)經(jīng)腹探查,按規(guī)范化序貫方法觀察胎兒整體解剖結(jié)構(gòu)并測量頸項透明層(NT)厚度,對陽性病例追蹤隨訪至產(chǎn)后,并將嚴(yán)重畸形結(jié)果與引產(chǎn)后病理做對照分析。結(jié)果發(fā)現(xiàn)畸形胎兒17例,包含畸形總數(shù)27例,其中脊柱側(cè)彎1例,無腦兒2例,露腦畸形1例,無葉全前腦1例,頸部水囊瘤6例,唇腭裂1例,胎兒水腫9例,心臟異常2例,臍膨出2例,下肢發(fā)育異常1例,巨膀胱1例,除2例心臟結(jié)構(gòu)異常外均經(jīng)引產(chǎn)證實。NT值≥3.0 mm 32例,其中染色體異常3例(羊水穿刺證實2例為21-三體綜合征、1例為18-三體綜合征)、頸部水囊瘤6例、全身水腫3例、胎死宮內(nèi)1例,余產(chǎn)后正常。另檢出5例鼻骨缺失,2例經(jīng)羊水穿刺證實為21-三體綜合征,1例胎死宮內(nèi),2例產(chǎn)后正常。結(jié)論早孕期規(guī)范化檢查可以較早發(fā)現(xiàn)胎兒嚴(yán)重結(jié)構(gòu)畸形,對產(chǎn)前診斷具有較大臨床應(yīng)用價值。NT值增厚對于早期評估染色體異常風(fēng)險及指導(dǎo)預(yù)后有顯著意義。
[Abstract]:Objective to investigate the application value of standardized ultrasonography in early pregnancy in fetal malformation screening. Methods A total of 2670 pregnant women were selected for early pregnancy screening at 11136-week. The fetal whole anatomical structure was observed and the thickness of the transparent layer of the neck was measured according to the standardized sequential method. The positive cases were followed up to the postpartum period. The results of severe malformation were compared with the pathology after induced labor. The results showed that there were 17 cases of malformed fetus, including 27 cases of malformation, including 1 case of scoliosis, 2 cases of anencephaly, 1 case of deformity, 1 case of anencephaly, 1 case of anencephaly, 6 cases of cervical hydrocystoma, 1 case of cleft lip and palate, 9 cases of fetal edema. There were 2 cases of cardiac abnormality, 2 cases of omphalocele, 1 case of lower extremity dysplasia, 1 case of giant bladder, 32 cases of which were proved by induced labor to be 鈮，
