【摘要】：目的探討輸卵管重復(fù)異位妊娠(REP)危險(xiǎn)因素及臨床特點(diǎn)。方法納入北京協(xié)和醫(yī)院2013年1月至2014年12月共78例REP組患者,并按首次異位妊娠(EP)年齡、婚姻狀態(tài)、就業(yè)狀態(tài),以1∶1比例匹配77例同期收治初次治療輸卵管異位妊娠(SEP)患者作為SEP組,比較兩組的臨床特點(diǎn),并分析REP的危險(xiǎn)因素。結(jié)果未生育(OR=2.163,95%CI 1.066~4.386)、既往EP開腹手術(shù)史(OR=8.585,95%CI 1.001~73.596)是REP的危險(xiǎn)因素。REP與SEP臨床表現(xiàn)相似,除了REP組術(shù)前超聲直徑4cm異位妊娠包塊數(shù)較SEP組明顯減少(P0.01)。REP組術(shù)中發(fā)現(xiàn)合并粘連及對(duì)側(cè)輸卵管病變的發(fā)生率明顯高于SEP組(P0.05)。結(jié)論積極防治首次異位妊娠、合理避孕、采用腹腔鏡手術(shù)可預(yù)防REP發(fā)生。
[Abstract]:Objective to investigate the risk factors and clinical features of repeated ectopic tubal pregnancy (RP).Methods from January 2013 to December 2014, 78 patients with REP were enrolled in Beijing Union Hospital. According to the age, marital status and employment status of the first ectopic pregnancy, the patients were enrolled in the study.77 patients with primary treatment of ectopic tubal pregnancy were treated at 1:1 as SEP group. The clinical characteristics of the two groups were compared and the risk factors of REP were analyzed.Results the risk factors of REP were 2.16395 CI 1.066 and 4.386.The history of EP open surgery was 8.58595 CI 1.001 / 73.596). The clinical manifestation of SEP was similar to that of rep.In addition, the number of ectopic 4cm ectopic pregnancy lumps in REP group was significantly lower than that in SEP group. The incidence of combined adhesion and contralateral fallopian tube lesions was significantly higher in the REP group than in the SEP group.Conclusion the prevention and treatment of first ectopic pregnancy, rational contraception and laparoscopic surgery can prevent REP.
