【摘要】：目的：探討免結(jié)扎技術(shù)在宮頸癌手術(shù)治療中的應(yīng)用價(jià)值。 方法：采用前瞻性隨機(jī)對照研究,通過設(shè)定入選標(biāo)準(zhǔn)與排除標(biāo)準(zhǔn),篩選自：2011年10月至2013年12月期間,因?qū)m頸癌在廣西壯族自治區(qū)人民醫(yī)院婦科住院,需行經(jīng)腹廣泛性全子宮切除術(shù)+盆腔淋巴結(jié)清掃術(shù)的病例共105例,隨機(jī)分為2組,其中53例術(shù)中應(yīng)用Li gasure血管閉合系統(tǒng),作為實(shí)驗(yàn)組；52例術(shù)中使用傳統(tǒng)縫扎技術(shù),作為對照組；所有兩組篩選出的患者術(shù)前均已確診宮頸癌并明確分期,未合并嚴(yán)重內(nèi)、外科疾病,未行術(shù)前放、化療等。收集記錄兩組所有患者的手術(shù)時(shí)間、術(shù)中出血量及住院時(shí)間等等術(shù)前術(shù)后情況,通過對比應(yīng)用Li gasure血管閉合系統(tǒng)的經(jīng)腹廣泛性全子宮切除術(shù)+盆腔淋巴結(jié)清掃術(shù)與對照組行此術(shù)式的以上指標(biāo)進(jìn)行分析比較。 結(jié)果：(1)手術(shù)時(shí)間：對照組完成手術(shù)時(shí)間為160～295min(207.15±35.66min),實(shí)驗(yàn)組完成手術(shù)時(shí)間為122～278min(162.92±35.61min),對照組手術(shù)時(shí)間明顯長于實(shí)驗(yàn)組(p0.05)；(1)術(shù)中出血量：對照組手術(shù)過程中的出血量為300～1800ml(720.19±352.90m1),實(shí)驗(yàn)組手術(shù)過程中的出血量為120～1500ml(591.51±286.57m1),對照組手術(shù)過程中的出血量明顯多于實(shí)驗(yàn)組(p0.05)；(3)住院時(shí)間：對照組住院時(shí)間為10-34天(16.85±5.73天),實(shí)驗(yàn)組住院時(shí)間為9-28天(15.20±4.05天),實(shí)驗(yàn)組與對照組住院時(shí)間比較,差異無顯著性(p=0.09)。 結(jié)論：免結(jié)扎技術(shù)(Ligasure血管閉合系統(tǒng))應(yīng)用在宮頸癌手術(shù)中,能明顯減少手術(shù)時(shí)間及術(shù)中出血量,但對患者住院時(shí)間的影響尚需進(jìn)一步對照研究。Ligasure血管閉合系統(tǒng)具有止血可靠、熱傳導(dǎo)小、體內(nèi)無異物存留等特點(diǎn),因其可容易快捷控制出血,避免了繁瑣的結(jié)扎和止血從而明顯縮短手術(shù)時(shí)間及減少術(shù)中出血量,可以在婦科其他腫瘤手術(shù)中推廣應(yīng)用。
[Abstract]:Objective : To evaluate the value of ligation - free technique in the treatment of cervical cancer .

Methods : From October 2011 to December 2013 , 105 patients were randomly divided into 2 groups . Among them , 53 cases were treated with Li gasure vascular closure system as experimental group .
Conventional suture ligation was used in 52 cases as control group .
All patients were diagnosed with cervical cancer before the procedure , without serious internal and surgical diseases , pre - operative radiotherapy , chemotherapy , etc . The operation time of all patients in the two groups , the amount of bleeding during operation and the time of hospitalization were collected and recorded , and compared with those of the control group , the above indexes were analyzed by comparing the total hysterectomy with the Li gasure vascular closure system plus pelvic lymph node dissection .
