【摘要】：目的：比較復(fù)發(fā)性流產(chǎn)患者外周血和蛻膜組織中TNF-α的表達水平。比較不同流產(chǎn)次數(shù)的復(fù)發(fā)性流產(chǎn)患者TNF-α水平。比較TNF-α對原發(fā)性、繼發(fā)性復(fù)發(fā)性流產(chǎn)的影響。通過對TNF-α的測定，為臨床治療復(fù)發(fā)性流產(chǎn)提供理論依據(jù)。 方法：選取2013.1.1～2014.3.1于大連醫(yī)科大學(xué)附屬大連市婦產(chǎn)醫(yī)院門診手術(shù)室行人工流產(chǎn)術(shù)的早期妊娠復(fù)發(fā)性流產(chǎn)患者76例作為病例組，同期正常妊娠者100例作為對照組。用ELISA方法測定兩組患者外周血和蛻膜組織中TNF-α的水平。同時比較不同流產(chǎn)次數(shù)的復(fù)發(fā)性流產(chǎn)患者TNF-α水平，以及TNF-α對原發(fā)性、繼發(fā)性復(fù)發(fā)性流產(chǎn)的影響。 結(jié)果：（1）復(fù)發(fā)性流產(chǎn)組患者的外周血中TNF-α表達水平明顯高于正常妊娠組，差異有統(tǒng)計學(xué)意義（p=0.000）；復(fù)發(fā)性流產(chǎn)組患者的蛻膜組織中TNF-α表達水平亦明顯高于對照組，差異有顯著統(tǒng)計學(xué)意義（p=0.000）。（2）2次復(fù)發(fā)性流產(chǎn)患者外周血中TNF-α表達水平與3次的比較，差異無統(tǒng)計學(xué)意義（p=0.200）；與3次以上的比較，差異有統(tǒng)計學(xué)意義（p=0.000）；3次與3次以上復(fù)發(fā)性流產(chǎn)患者TNF-α表達水平比較，，差異有統(tǒng)計學(xué)意義（p=0.018）。（3）2次復(fù)發(fā)性流產(chǎn)患者蛻膜組織中TNF-α表達水平與3次的比較，差異無統(tǒng)計學(xué)意義（p=0.256）；與3次以上的比較，差異有統(tǒng)計學(xué)意義（p=0.001）；3次與3次以上的比較，差異有統(tǒng)計學(xué)意義（p=0.014）。（4）原發(fā)性RSA組患者的外周血中TNF-α表達水平明顯低于繼發(fā)性RSA組，差異有顯著統(tǒng)計學(xué)意義（p=0.000）；繼發(fā)性RSA組患者的蛻膜組織中TNF-α表達水平明顯高于原發(fā)性RSA組，差異有統(tǒng)計學(xué)意義（p=0.002）。 結(jié)論：（1）TNF-α在復(fù)發(fā)性流產(chǎn)患者外周血和蛻膜組織中的表達均異常升高，其可能為導(dǎo)致RSA發(fā)生的病因之一。TNF-α在外周血和蛻膜組織中的表達一致，外周血中檢測到的TNF-α水平能同時反映局部組織中的水平。（2）RSA發(fā)生3次及以上，TNF-α表達水平隨著流產(chǎn)次數(shù)增加而增高，說明其可能對此次妊娠結(jié)局有重要參考價值，可作為臨床預(yù)測RSA發(fā)生的指標。（3）繼發(fā)性RSA患者外周血和蛻膜組織中的TNF-α表達水平均顯著高于原發(fā)性RSA患者，說明TNF-α對繼發(fā)性RSA的發(fā)生影響更大。
[Abstract]:Objective: to compare the expression of TNF- 偽 in peripheral blood and decidua of recurrent abortion patients.The levels of TNF- 偽 in recurrent abortion patients with different abortion times were compared.To compare the effect of TNF- 偽 on primary and secondary recurrent abortion.The determination of TNF- 偽 provides a theoretical basis for clinical treatment of recurrent abortion.Methods: 76 cases of early pregnancy recurrent abortion were selected as case group and 100 cases of normal pregnancy as control group, which were performed in the operating room of Dalian Obstetrics and Gynecology Hospital affiliated to Dalian Medical University on January 1, 2014.2014.The patients with recurrent early pregnancy were selected as the case group.The levels of TNF- 偽 in peripheral blood and decidua were measured by ELISA.At the same time, the levels of TNF- 偽 and the effect of TNF- 偽 on primary and secondary recurrent abortion were compared.Results the expression of TNF- 偽 in peripheral blood of recurrent abortion group was significantly higher than that of normal pregnancy group, and the expression of TNF- 偽 in decidual tissue of recurrent abortion group was significantly higher than that of control group.The expression of TNF- 偽 in peripheral blood of patients with recurrent abortion was significantly higher than that of patients with recurrent abortion. There was no significant difference in the expression of TNF- 偽 between the patients with recurrent abortion and three times, but there was no significant difference in the expression of TNF- 偽 between the patients with recurrent abortion and the patients with recurrent abortion more than 3 times.The expression of TNF- 偽 in decidua of patients with recurrent abortion was significantly higher than that in patients with recurrent abortion for 3 times and more than 3 times, and the expression level of TNF- 偽 in decidua of patients with recurrent abortion was significantly higher than that in patients with recurrent abortion for 3 times, and the expression level of TNF- 偽 in decidua of patients with recurrent abortion was significantly higher than that in patients with recurrent abortion for more than 3 times.The expression of TNF- 偽 in decidua in secondary RSA group was significantly higher than that in primary RSA group, and the difference was statistically significant.Conclusion the expression of TNF- 偽 in peripheral blood and decidua of patients with recurrent abortion is abnormally high, which may be one of the causes of RSA. The expression of TNF- 偽 in peripheral blood and decidua is consistent.The level of TNF- 偽 detected in peripheral blood can reflect the level of TNF- 偽 in local tissues at the same time. The expression level of TNF- 偽 increases with the number of miscarriages, which indicates that TNF- 偽 expression may be of important reference value to the outcome of pregnancy.The expression of TNF- 偽 in peripheral blood and decidua in patients with secondary RSA was significantly higher than that in patients with primary RSA, indicating that TNF- 偽 had more influence on the occurrence of secondary RSA.
