【摘要】：目的分析探討子宮內(nèi)膜異位癥(EMT)腹腔鏡治療后行體外受精-胚胎移植(IVF-ET)的妊娠時(shí)機(jī)和影響因素。方法選取診斷為EMT經(jīng)腹腔鏡治療后需行IVF-ET治療的120例不育癥患者為研究對(duì)象,按照患者術(shù)后妊娠時(shí)間1年(A組)、2年(B組)及3年(C組)進(jìn)行分組,觀察3組患者術(shù)后妊娠情況。分析行IVF-ET治療的不育癥患者的妊娠率與術(shù)后時(shí)間的關(guān)系。選用年齡、是否使用促性腺激素釋放激素激動(dòng)劑(GnRH-a)及EMT分期進(jìn)行多因素logistic回歸,分析其妊娠相關(guān)的影響因素。結(jié)果隨訪患者術(shù)后第1、2、3年(行IVF-ET治療)妊娠率分別為55.00%、32.50%、25.00%;術(shù)后第1年與術(shù)后第2年及第3年妊娠率比較,差異均有統(tǒng)計(jì)學(xué)意義(P0.05);單因素分析結(jié)果發(fā)現(xiàn),患者的年齡、術(shù)后是否使用避孕措施、術(shù)后GnRH-a使用情況和EMT分期與術(shù)后1年內(nèi)患者妊娠情況有明顯相關(guān)性(P0.05);多因素條件logistic回歸分析顯示,高齡育齡期女性、術(shù)后是否使用避孕措施、術(shù)后使用GnRH-a和EMT分期與術(shù)后1年內(nèi)妊娠情況有關(guān)(P0.05)。結(jié)論 EMT腹腔鏡治療后行IVF-ET的妊娠時(shí)機(jī)與多種因素有關(guān),以第1年內(nèi)妊娠率最高,且高齡育齡期女性、術(shù)后使用GnRH-a和EMT分期與術(shù)后1年內(nèi)輔助妊娠情況有明顯相關(guān)性。
[Abstract]:Objective to analyze the pregnancy timing and influencing factors of IVF-ETT after laparoscopic treatment of endometriosis. Methods 120 infertile patients who were diagnosed as EMT who needed IVF-ET treatment after laparoscopic treatment were selected as the study objects. According to the time of pregnancy 1 year after operation, group B (2 years) and group C (3 years), the pregnancy status of the three groups was observed, and the relationship between pregnancy rate and postoperative time of infertility treated with IVF-ET was analyzed. Whether gonadotropin releasing hormone agonist (GnRH-a) and EMT staging were used for multivariate logistic regression, Results the pregnancy rates in the first and third year (IVF-ET treatment) were 55.00 and 32.50 and 25.00, respectively, and the pregnancy rates in the first year and the second and the third year after operation were compared. The difference was statistically significant (P 0.05). Univariate analysis showed that the age of the patients and whether or not to use contraception after operation, There was a significant correlation between postoperative GnRH-a use and EMT stage and pregnancy status within 1 year after operation (P 0.05). Multivariate conditional logistic regression analysis showed that the elderly women of childbearing age were using contraception after operation. GnRH-a and EMT stages were associated with pregnancy within 1 year after operation (P 0.05). Conclusion the pregnancy timing of IVF-ET after EMT laparoscopic therapy is related to many factors, and the pregnancy rate is the highest in the first year, and the elderly women of childbearing age have the highest pregnancy rate. The use of GnRH-a and EMT stages was significantly correlated with the degree of assisted pregnancy within 1 year after operation.
