【摘要】：目的研究子宮內(nèi)膜細(xì)胞取材器診斷慢性子宮內(nèi)膜炎的價(jià)值。 方法（1）選取患者先行子宮內(nèi)膜細(xì)胞采集器取樣后行診斷性刮宮術(shù)取樣。標(biāo)本分為子宮內(nèi)膜細(xì)胞取材器組和診斷性刮宮術(shù)組，，兩組取材結(jié)果滿意率的比較；（2）免疫組織化學(xué)方法測(cè)CD38、CD138均陽(yáng)性時(shí)可診斷慢性子宮內(nèi)膜炎。將子宮內(nèi)膜細(xì)胞取材器組獲取的標(biāo)本分為HE染色法組與免疫組織化學(xué)染色法檢測(cè)CD38、CD138組，兩組病理結(jié)果診斷慢性子宮內(nèi)膜炎陽(yáng)性率的比較；（3）子宮內(nèi)膜細(xì)胞取材器組和診斷性刮宮術(shù)組，行免疫組織化學(xué)染色法檢測(cè)CD38、CD138，兩組病理結(jié)果診斷慢性子宮內(nèi)膜炎陽(yáng)性率的比較。（1）、（2）、（3）采用配對(duì)四格表資料2檢驗(yàn)。 結(jié)果（1）診刮術(shù)組取材滿意率（92.5%）與取材器組取材滿意率（88%）比較差異無(wú)統(tǒng)計(jì)學(xué)意義（2=1.64，P=0.2008）。（2）免疫組織化學(xué)染色法檢測(cè)CD38、CD138組診斷陽(yáng)性率（26.19%）與HE染色組診斷陽(yáng)性率（3.57%）比較差異有統(tǒng)計(jì)學(xué)意義（2=36，P0.005）。（3）子宮內(nèi)膜細(xì)胞取材器組診斷陽(yáng)性率（26.19%）與診刮術(shù)組診斷陽(yáng)性率（27.38%）比較差異無(wú)統(tǒng)計(jì)學(xué)意義（2=0.083，P0.5）。 結(jié)論（1）子宮內(nèi)膜細(xì)胞取材器可取到滿意的組織學(xué)標(biāo)本；（2）采用免疫組織化學(xué)方法檢測(cè)CD38聯(lián)合CD138較傳統(tǒng)HE染色法診斷慢性子宮內(nèi)膜炎方法更具靈敏性和特異性；（3）子宮內(nèi)膜細(xì)胞取材器所獲標(biāo)本對(duì)慢性子宮內(nèi)膜炎診斷結(jié)果可靠、準(zhǔn)確性較高。
[Abstract]:Objective to study the value of endometrial cell sampler in the diagnosis of chronic endometritis. Methods 1) the specimens were divided into two groups: endometrial cell sampling group and diagnostic curettage group. Comparison of satisfactory rate of results obtained from two groups: (2) Immunohistochemical method can diagnose chronic endometritis when CD38 + CD138 is positive. The specimens obtained from endometrial cells were divided into HE staining group and immunohistochemistry CD38 + CD138 group. A comparison of the positive rates of pathological results in the diagnosis of chronic endometritis: the endometrial cell sampling apparatus group and the diagnostic curettage group, Immunohistochemical staining was used to detect CD38 and CD138.The positive rates of CD38 and CD138were compared between the two groups in the diagnosis of chronic endometritis. Results 1) there was no significant difference in the satisfaction rate between the curettage group (92.5) and the sampler group (88%). There was no significant difference between the two groups. There was no significant difference between the two groups. (2) Immunohistochemical staining was used to detect the diagnostic positive rate of CD38 + CD138 group (26.19%) and HE staining group (3.57%). The positive rate of diagnosis in the endometrial cell sampling group was 26.19) and that in the curettage group was 27.38) there was no significant difference between the two groups. Conclusion 1) A satisfactory histological specimen can be obtained from endometrial cell sampler. The detection of CD38 combined with CD138 by immunohistochemical method is more sensitive and specific than the traditional HE staining method in the diagnosis of chronic endometritis. The specimens obtained from the endometrial cell extractor were reliable for the diagnosis of chronic endometritis. The accuracy is high.
