【摘要】：目的探討陰道乳桿菌活菌制劑定君生與抗真菌藥物聯合及序貫應用治療重度外陰陰道假絲酵母菌病(VVC)的有效性及復發(fā)情況。方法選取北京大學第一醫(yī)院2015年10月至2016年1月收治的125例重度VVC患者,采用隨機數字表法分為3組,即聯合組(46例)、序貫組(39例)、單純用藥組(40例),其中聯合組為定君生與氟康唑同時聯合用藥,序貫組為采用氟康唑先行治療隨后序貫應用定君生,單純用藥組僅應用氟康唑治療。3組均在治療開始后第14~21天進行第1次復查,第28~35天進行第2次復查,觀察兩次復查后的有效率及復發(fā)率。結果第1次復查時聯合組、序貫組、單純用藥組有效率分別為79.1%、83.3%、42.5%,聯合組與序貫組差異無統計學意義,與單純用藥組差異有統計學意義(P0.05);第2次復查時有效率分別為75.6%、59.0%、27.5%,聯合組與其他兩組比較差異有統計學意義(P0.05);復發(fā)率為0、0、12.5%,3組間差異有統計學意義(P0.05)。結論乳桿菌制劑與氟康唑聯合及序貫用藥治療重度VVC均可達到更高的有效率并減低復發(fā)率,聯合用藥效果更佳。
[Abstract]:Objective to investigate the efficacy and recurrence of Dingjunsheng and antifungal drugs combined with Lactobacillus vaginalis in the treatment of severe vulvovaginal candidiasis. Methods in October 2015, the first Hospital of Peking University was selected. By January 2016, 125 patients with severe VVC had been treated. The method of random digital table was used to divide into three groups: combined group (n = 46), sequential group (n = 39), single drug group (n = 40). The combined group was treated with fluconazole and fluconazole at the same time, and the sequential group was treated with fluconazole first and then followed by Dingjunsheng. The group treated with fluconazole was treated with fluconazole for the first time on the 21st day after treatment and the second for the 2835th day. The effective rate and recurrence rate were observed. Results the combined group and sequential group were used in the first reexamination. The effective rates were 79.1% and 83.3%, respectively. There was no significant difference between the combined group and the sequential group. The effective rate of the second reexamination was 75.6% and 59.0%, respectively. The difference between the combined group and the other two groups was statistically significant (P 0.05), and the recurrence rate was 0 0 / 12. 5%. Conclusion there is a significant difference among the three groups in the recurrence rate of Lactobacillus preparation. In combination with fluconazole and sequential treatment of severe VVC, the effective rate was higher and the recurrence rate was reduced. The effect of combined medication is better.
