【摘要】：目的探討卵巢成熟型囊性畸胎瘤惡性病變患者的臨床病例特征與基因檢測(cè)結(jié)果。方法回顧性選取2014年至2017年收治的8例卵巢成熟型囊性畸胎瘤惡性病變患者,對(duì)患者的臨床病理檢查結(jié)果與血清基因檢查結(jié)果進(jìn)行分析。結(jié)果 8例患者均為單側(cè)卵巢病變,下腹部疼痛的患者為3例。腫瘤最大徑在10 cm以上的患者3例。8例患者中鱗癌3例,腺癌5例。鱗癌影像學(xué)檢查囊壁存在部分厚度增加,腺癌存在附壁乳頭組織,免疫組化檢查結(jié)果中鱗癌P63與CK5/6為陽(yáng)性,腺癌Villin、CK20、CDX2為陽(yáng)性�；驒z測(cè)發(fā)現(xiàn)腸狀腺癌惡性病變患者存在KRAS基因突變。結(jié)論通過(guò)臨床檢查、超聲影像學(xué)檢查以及血清腫瘤標(biāo)記因子可以診斷部分卵巢成熟型囊性畸胎瘤惡性病變,KRAS基因突變檢查可以輔助診斷是否存在惡性腫瘤病變。
[Abstract]:Objective to investigate the clinical features and gene detection results of ovarian mature cystic teratoma malignant lesions. Methods eight patients with ovarian mature cystic teratoma from 2014 to 2017 were retrospectively selected. The results of clinicopathological examination and serum gene examination were analyzed. There were 3 patients with pain in the lower abdomen, 3 patients with squamous cell carcinoma and 5 patients with adenocarcinoma with the largest tumor diameter over 10 cm. The cystic wall of squamous cell carcinoma was partly thickened and the papilla tissue attached to the wall was found in adenocarcinoma. The results of immunohistochemical examination showed that p63 and CK5/6 were positive in squamous cell carcinoma, and Villin CK20 CDX2 was positive in adenocarcinoma. The mutation of KRAS gene was found in malignant lesions of intestinal adenocarcinoma by gene detection. Ultrasound imaging and serum tumor markers can be used to diagnose malignant lesions of ovarian mature cystic teratoma.
