血清中γ-GT及GSH在子癇前期患者中的表達及意義

【摘要】:目的:觀察子癇前期輕度及重度患者血清中γ-谷氨酰轉肽酶(gamma-glutamyltransferase, γ-GT)及谷胱甘肽(γ-谷氨酰-L半胱氨酸-L-甘氨酸,glutathione, GSH)的表達,以明確其與子癇前期的相關性及意義。 方法:隨機選取63例妊娠期高血壓疾病患者,根據人民衛(wèi)生出版社出版的婦產科學第七版教材中妊高癥的診斷,將其分為子癇前期重度組31例,子癇前期輕度組32例,以28例同孕周正常妊娠者為對照組,空腹抽外周血,用ELISA法檢測血清中GSH水平,用全自動生化分析儀檢測血清中γ-GT、血脂、肝功、腎功水平及24-小時尿蛋白,同時觀察血壓、年齡、孕周等相關指標,比較這些指標在子癇前期重度(sPE組)、輕度(mPE組)和正常妊娠組(Normal)表達的差異。 結果:1.三組年齡、孕周均無顯著差異(P0.05)。2.各研究組之間γ-GT、GSH的比較結果:①子癇前期重度組及輕度組血清中γ-GT含量均明顯高于正常妊娠組,GSH含量均明顯低于正常妊娠組(P0.05)。②子癇前期重度組γ-GT含量高于輕度組,GSH含量低于輕度組(P0.05)。3.研究組之間血脂的比較結果:①子癇前期重度組與正常組相比,重度組中總膽固醇(CHO)、甘油三酯(TG)、低密度脂蛋白(LDL)的含量明顯升高,高密度脂蛋白(HDL)明顯降低(P0.05)。②子癇前期輕度組與正常組相比,輕度組低密度脂蛋白(LDL)高于正常組,高密度脂蛋白(HDL)低于正常組(P0.05),總膽固醇(CHO)、甘油三酯(TG)與正常組無明顯差異(P0.05)。③子癇前期重度組與輕度組相比,重度組中的總膽固醇(CHO)、甘油三酯(TG)、低密度脂蛋白(LDL)含量均高于輕度組,高密度脂蛋白(HDL)低于輕度組(P0.05)。4.根據Pearson相關分析發(fā)現,1,-GT與收縮壓、舒張壓、谷丙轉氨酶(ALT)、谷草轉氨酶(AST)、總膽固醇(CHO)、低密度脂蛋白(LDL)、血尿酸(UA)、24-小時尿蛋白(24-hour proteinuria)表現為正相關關系(P0.01),但與高密度脂蛋白(HDL)、GSH呈負相關關系(P0.01)。 結論:子癇前期患者中存在不同程度的異常血脂代謝。γ-GT在子癇前期患者中的表達隨著疾病的嚴重程度升高,GSH隨著疾病的嚴重程度降低,二者均與子癇前期具有相關性,可作為評估子癇前期嚴重程度的指標,運用于臨床判斷疾病的嚴重程度。
[Abstract]:Aim: to investigate the expression of gamma-glutamyltransferase (緯 -GT) and 緯 -glutamyltransferase (緯 -GT) and glutathione (GSH) in the serum of mild and severe preeclampsia patients, and to determine the correlation and significance between 緯 -glutamyl transpeptidase (緯 -GT) and preeclampsia. Methods: Sixty-three patients with hypertensive disorder complicating pregnancy were randomly selected. According to the diagnosis of PIH in the teaching material of 7th edition of obstetrics and gynecology published by the people's Health Publishing House, they were divided into three groups: severe preeclampsia group (31 cases) and mild preeclampsia group (32 cases). 28 normal pregnant women of the same gestational age were taken as control group. The serum GSH level was detected by ELISA method, serum 緯 -GT, lipids, liver function, renal function and 24-hour urine protein were detected by automatic biochemical analyzer, and blood pressure was observed at the same time. Age, gestational age and other related indexes were compared between severe preeclampsia group (sPE group), mild group (MPE group) and normal pregnancy group (normal group). Result: 1.Three groups of ages, There was no significant difference in gestational age between the two groups. The results showed that the serum 緯 -GT levels in the severe preeclampsia group and mild eclampsia group were significantly higher than those in the normal pregnancy group and that in the normal pregnancy group were significantly lower than those in the normal pregnancy group and the normal pregnancy group. 2. The results showed that the level of 緯 -GT in the serum of the severe preeclampsia group was significantly lower than that in the normal pregnancy group. The content of 緯 -GT was higher in mild group than that in mild group (P 0.05) .3.The comparison of blood lipids between study group and severe preeclampsia group was compared with that in normal group. The contents of total cholesterol, triglyceride, low density lipoprotein (LDL) and high density lipoprotein (HDL) decreased significantly in the severe group compared with the normal group, and the low density lipoprotein (LDL) level in the mild group was significantly higher than that in the normal group, and that in the mild group of mild preeclampsia was significantly lower than that in the normal group. High density lipoprotein (HDL) was lower than normal group (P 0.05), total cholesterol and triglyceride (TG) were not significantly different from normal group (P 0.05.3) compared with mild group and severe preeclampsia group (P 0.05.3), there was no significant difference between severe preeclampsia group (P 0.05) and mild group (P < 0.05). The contents of total cholesterol, triglyceride, low density lipoprotein (LDLs) in the severe group were higher than those in the mild group, and HDLs were lower than those in the mild group (P0.05. 4). According to the correlation analysis of Pearson, it was found that the contents of TTG, TG, LDLs were higher than those in the mild group, and the results of Pearson correlation analysis showed that they were associated with systolic blood pressure (SBP) and diastolic blood pressure (DBP). Alanine aminotransferase (alt), alanine aminotransferase (alt), glutamic oxalacetic transaminase (AST), total cholesterol (TC), low density lipoprotein (LDLN), 24-hour urinary protein (24-hour) were positively correlated, but negatively correlated with high density lipoprotein (HDLGSH) (P 0. 01). Conclusion: there are abnormal lipid metabolism in preeclampsia patients, and the expression of 緯 -GT in preeclampsia patients decreases with the severity of the disease, both of which are correlated with preeclampsia. It can be used as an index to evaluate the severity of preeclampsia.
