【摘要】：目的隨著左炔諾孕酮宮內緩釋系統(tǒng)(levonorgestrel releasing intrauterine system,LNG-IUS)適應證的不斷擴展,由其引起的一系列不良反應也逐漸受到重視。文中通過提供參考觀察LNG-IUS治療子宮腺肌病中的重要不良反應,對干預性處理的可行性進行探討,為其減少LNG-IUS不良反應、增加其使用率和使用年限、提高治療效果。方法回顧性分析2012年1月至2013年12月期間在桂林醫(yī)學院附屬醫(yī)院診斷為子宮腺肌病并自愿接受LNG-IUS治療的67例患者作為研究對象,根據不同處理方法納入干預組(n=34)和對照組(n=33)。干預組放置LNG-IUS后立即予干預性處理,即口服非甾體抗炎藥吲哚美辛25 mg,1片/次、3次/d,連服7 d,同時口服去氧孕烯炔雌醇1次/d、1片/次,連服21 d,連用3個周期;對照組僅予病情溝通未做特殊處理。分別觀察和記錄治療后1、3、6個月相關不良反應。結果經干預性處理后,干預組1、3、6個月的子宮異常出血發(fā)生率分別為8例(23.5%)、4例(11.7%)、0例(0%);而對照組發(fā)生率分別為20例(60.6%)、12例(36.4%)、4例(12.1%)。組間各月比較差異均有統(tǒng)計學意義(P0.05)。觀察至6個月,閉經的總發(fā)生率為干預組8例(23.5%),對照組2例(6.1%),組間比較差異有統(tǒng)計學意義(P0.05);卵巢囊腫總發(fā)生率為干預組1例(2.9%),對照組7例(21.2%),差異有統(tǒng)計學意義(P0.05);環(huán)脫落或環(huán)移位總發(fā)生率干預組1例(2.9%),對照組6例(18.2%),差異有統(tǒng)計學意義(P0.05)。結論子宮腺肌癥患者放置LNG-IUS后立即予口服非甾體抗炎藥和短效避孕藥的干預性處理可明顯降低放置后子宮異常出血和其他不良反應的發(fā)生率,臨床應用具有較好的可行性。
[Abstract]:With the purpose of levonorgestrel releasing intrauterine system (levonorgestrel releasing intrauterine system, LNG-IUS) indication expansion, a series of adverse reactions caused by the gradually attention. Provide reference for an important observation of the adverse reaction of LNG-IUS treatment of adenomyosis in the text, to explore the feasibility of interventional treatment, to reduce adverse reactions LNG-IUS, increase the usage rate and service life, improve the therapeutic effect.
