【摘要】：目的：分析大連地區(qū)5年來不同級(jí)別助產(chǎn)機(jī)構(gòu)剖宮產(chǎn)狀況及剖宮產(chǎn)的主要影響因素，為衛(wèi)生行政部門提供決策支持，并為進(jìn)一步開展前瞻性的干預(yù)研究提供科學(xué)依據(jù)。 方法：將大連市助產(chǎn)機(jī)構(gòu)按照省、市、縣區(qū)、鄉(xiāng)鎮(zhèn)等分為4個(gè)級(jí)別，對(duì)2009—2013年在各級(jí)機(jī)構(gòu)住院分娩產(chǎn)婦的分娩方式進(jìn)行統(tǒng)計(jì)匯總，對(duì)剖宮產(chǎn)狀況及剖宮產(chǎn)影響因素進(jìn)行分析。 結(jié)果：2009—2013年大連市助產(chǎn)機(jī)構(gòu)共接產(chǎn)268281人次，順產(chǎn)123616人次，占46.08%；剖宮產(chǎn)137691人次，占51.32%；陰道手術(shù)助產(chǎn)6974人次，占2.60%。2009-2013年大連市助產(chǎn)機(jī)構(gòu)住分娩產(chǎn)婦數(shù)以縣區(qū)、地市兩級(jí)構(gòu)成比最多,選擇在地市級(jí)機(jī)構(gòu)分娩的產(chǎn)婦逐漸增加，在鄉(xiāng)鎮(zhèn)級(jí)機(jī)構(gòu)分娩的有所減少。5年來全市剖宮產(chǎn)率無明顯變化，維持在50-52%之間。從機(jī)構(gòu)級(jí)別來看剖宮產(chǎn)率由省級(jí)、縣區(qū)、鄉(xiāng)鎮(zhèn)級(jí)、地市級(jí)依次降低，但仍高于發(fā)達(dá)國家。鄉(xiāng)鎮(zhèn)級(jí)剖宮產(chǎn)率呈現(xiàn)出逐年上升的趨勢，省級(jí)和地市級(jí)助產(chǎn)機(jī)構(gòu)的剖宮產(chǎn)率呈現(xiàn)出平穩(wěn)中有所下降的趨勢，縣區(qū)級(jí)變化不大。從剖宮產(chǎn)率≥50%的機(jī)構(gòu)構(gòu)成比來看，鄉(xiāng)鎮(zhèn)級(jí)機(jī)構(gòu)剖宮產(chǎn)率≥50%的比例最少，鄉(xiāng)鎮(zhèn)、縣區(qū)兩級(jí)機(jī)構(gòu)剖宮產(chǎn)率≥50%的機(jī)構(gòu)比例都呈逐年增加趨勢。 結(jié)論：1.大連市2009年-2013年剖宮產(chǎn)率總體高居不下，均在50%左右，已遠(yuǎn)遠(yuǎn)高于15％的世界平均水平。2.近5年大連市剖宮產(chǎn)率總體變化不大，剖宮產(chǎn)率呈現(xiàn)省級(jí)、縣區(qū)級(jí)最高，而鄉(xiāng)鎮(zhèn)級(jí)上升較快的態(tài)勢。3.剖宮產(chǎn)率的影響因素是多方面的，社會(huì)因素為近年來大連市剖宮產(chǎn)率顯著升高的主要因素，應(yīng)減少社會(huì)因素對(duì)分娩的干擾，從而達(dá)到降低剖宮產(chǎn)率。
[Abstract]:Objective: to analyze the situation of cesarean section and the main influencing factors of cesarean section in different levels of midwifery in Dalian area during the past 5 years, to provide decision support for health administration department, and to provide scientific basis for further prospective intervention research. Methods: according to province, city, county, township and so on, the midwifery organization in Dalian was divided into 4 levels. The delivery mode of hospitalized parturient in all levels from 2009 to 2013 was statistically collected, and the condition of cesarean section and the influencing factors of cesarean section were analyzed. Results from 2009 to 2013, the number of births in Dalian was 268281, 123616 (46.08), 137691 (51.3232) by cesarean section and 6974 (2.60.2009-2013) by vaginal surgery. The number of parturient in Dalian from 2009 to 2013 was county and district, and the ratio between prefectural and municipal levels was the highest. The number of women who choose to give birth at the prefectural and municipal level has gradually increased, and the number of births at the township level has decreased. In the past five years, the cesarean section rate in the whole city has not changed significantly, and it has remained between 50-52%. From the institutional level, the cesarean section rate is at the provincial, county, district, township, and township levels. The rate of cesarean section at the township level showed an increasing trend year by year, and the cesarean section rate of the provincial and municipal midwifery institutions showed a steady downward trend.
