【摘要】：目的:評(píng)價(jià)TROPHY硬型宮腔鏡系統(tǒng)在不孕癥患者門診無(wú)麻醉宮腔鏡檢查治療中的應(yīng)用價(jià)值。方法:選擇2016年4月至8月在北京大學(xué)第三醫(yī)院生殖醫(yī)學(xué)中心門診行宮腔鏡檢查并符合納入標(biāo)準(zhǔn)的200例不孕癥患者。隨機(jī)分為兩組,應(yīng)用TROPHY硬型宮腔鏡為硬鏡組(100例),使用軟型纖維宮腔鏡為軟鏡組(100例)。比較兩組患者的疼痛評(píng)分、檢查時(shí)間、置鏡失敗率、陰道窺器使用率、宮頸鉗使用率、并發(fā)癥發(fā)生率等。結(jié)果:兩組檢查時(shí)間、疼痛評(píng)分、置鏡失敗率、并發(fā)癥發(fā)生率比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。硬鏡組陰道窺器和宮頸鉗使用率較軟鏡組低,差異有統(tǒng)計(jì)學(xué)意義(P0.01)。TROPHY硬型宮腔鏡手術(shù)鞘完成子宮內(nèi)膜息肉的剪除和宮腔粘連分離術(shù)36例,其中12例由軟型宮腔鏡組轉(zhuǎn)硬型宮腔鏡完成。結(jié)論:TROPHY硬型宮腔鏡可以應(yīng)用于不孕患者無(wú)麻醉門診檢查中,并能完成部分手術(shù),患者耐受性好。
[Abstract]:Objective: to evaluate the value of TROPHY hard hysteroscopy system in the treatment of infertility patients without anesthesia hysteroscopy. Methods: from April 2016 to August, we chose to perform uterine cavity in the outpatient clinic of Reproductive Medicine Center of the third Hospital of Peking University. Two hundred infertile patients who were examined under microscope and met the inclusion criteria were randomly divided into two groups. TROPHY hard hysteroscopy was used as hard endoscopy group and soft fiber hysteroscopy as soft endoscope group. The pain score, examination time, failure rate, vaginal speculum usage rate and cervical forceps utilization rate were compared between the two groups. Results: there was no significant difference between the two groups in the examination time, pain score, failure rate of endoscopy and incidence of complications (P 0.05). The utilization rate of vaginal speculum and cervical forceps in hard mirror group was lower than that in soft microscope group. The difference was statistically significant (P 0.01) .TROPHY hard hysteroscopic surgical sheath was performed in 36 cases of endometrial polyps. Among them, 12 cases were treated by soft hysteroscopy group and hard hysteroscopy. Conclusion: TROPHY hard hysteroscopy can be used in the non-anaesthesia outpatient examination of infertile patients, and partial operation can be completed, and the patient has good tolerance.
