孕婦人群中無(wú)創(chuàng)式血紅蛋白檢測(cè)準(zhǔn)確性研究

【摘要】:目的探討孕婦人群中無(wú)創(chuàng)式檢測(cè)血紅蛋白(Hb)結(jié)果與靜脈血結(jié)果的差異和臨床意義,為孕婦的貧血篩查提供參考依據(jù)。方法采用無(wú)創(chuàng)式脈搏血氧血紅蛋白儀Or Sense NBM-200檢測(cè)2014年1月同濟(jì)大學(xué)附屬第一婦嬰保健院180例孕婦血紅蛋白(脈搏血氧血紅蛋白,Sp Hb),并同時(shí)檢測(cè)其靜脈血紅蛋白(venous Hb,v Hb),將結(jié)果進(jìn)行比較,評(píng)估其準(zhǔn)確性、相關(guān)性及一致性。結(jié)果在180例孕婦中Sp Hb與v Hb的差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.463)。Peason相關(guān)分析顯示,Sp Hb與v Hb相關(guān)系數(shù)為0.789(P0.001),表明兩種方式檢測(cè)結(jié)果有明顯相關(guān)關(guān)系。用Bland-Altman分析Sp Hb與v Hb的差值,結(jié)果顯示在不貧血孕婦中,平均偏倚為6.6 g/L,95%的一致性界限中的最大差值15.2 g/L為兩方式的均值120.0 g/L的12.7%;輕度貧血孕婦中,平均偏倚為7.0 g/L,95%的一致性界限中的最大差值16.6 g/L為兩方式的均值108.1 g/L的15.4%;中度貧血孕婦中,平均偏倚為6.2 g/L,95%的一致性界限中的最大差值20.2 g/L為兩方式的均值95.6 g/L的21.1%,表明兩種檢測(cè)方法在不同貧血程度的孕婦中均具有良好的一致性,但其準(zhǔn)確性隨著貧血程度的加重而降低。結(jié)論使用NBM-200檢測(cè)儀得到的Sp Hb與v Hb具有較好的一致性和相關(guān)性,雖然準(zhǔn)確性有待進(jìn)一步提升改進(jìn),但其具有無(wú)創(chuàng)傷和快速的優(yōu)點(diǎn),可以更好地為孕婦進(jìn)行貧血篩查。
[Abstract]:Objective to explore the difference and clinical significance between the results of noninvasive hemoglobin test and venous blood in pregnant women. Methods the non-invasive pulse hemoglobin (or Sense NBM-200) was used to detect the hemoglobin (pulse hemoglobin) of 180 pregnant women in the first maternal and infant health care hospital affiliated to Tongji university in January 2014. The venous hemoglobin was detected at the same time and the results were compared. To assess its accuracy, Results there was no significant difference between SB and vHb in 180 pregnant women. Peason correlation analysis showed that the correlation coefficient between SB and vHb was 0.789 and P0.001, which indicated that there was a significant correlation between the two methods. Bland-Altman analysis showed that there was a significant correlation between the two methods. The difference between SB and v HB, The results showed that the maximum difference of 15.2 g / L was 12.7g / L of 120.0 g / L in non-anemia pregnant women with an average bias of 6.6 g / L 95%. The maximum difference in the consistency limit of an average bias of 7.0 g / L 95% was 16.6 g / L, which was the two-way average of 108.1 g / L 15.4g / L; in moderately anaemic pregnant women, The maximum difference of 20.2 g / L in the consistency limit with an average bias of 6.2 g / L 95% was the average of 95.6 g / L in two ways, indicating that the two methods had good consistency among pregnant women with different levels of anemia. Conclusion the accuracy of SpHb and vHb obtained by using NBM-200 detector has good consistency and correlation. Although the accuracy needs to be further improved, it has the advantages of non-invasive and rapid. Better screening for anaemia in pregnant women.
