【摘要】：目的探討抗苗勒管激素(Anti-mullerian hormone,AMH)在高齡婦女卵巢功能評估中的價(jià)值。方法收集2014年10月-2015年2月在柳州市婦幼保健院門診接受咨詢的高齡婦女100例,根據(jù)其月經(jīng)狀態(tài)將其分正常組32例,圍絕經(jīng)期組35例,卵巢早衰(Premature Ovarian Failure,POF)組33例,比較各組的黃體生成素(Luteinizing Hormone,LH)、卵泡刺激素(Follicle-Stimulating Hormone,FSH)、雌二醇(Estradiol,E2)和AMH水平,分析圍絕經(jīng)期組女性入組時(shí)、6個(gè)月、12個(gè)月時(shí)LH、FSH、E2和AMH水平變化,對比隨訪自然妊娠女性與非妊娠女性在入組時(shí)的LH、FSH、E2和AMH水平差異。結(jié)果圍絕經(jīng)期組血清FSH水平明顯高于正常組(P0.05),AMH水平明顯低于正常組(P0.05);POF組LH、FSH水平明顯高于正常組和圍絕經(jīng)期組(P0.05),E2、AMH水平明顯低于正常組和圍絕經(jīng)期組(P0.05)。圍絕經(jīng)期組入組后6個(gè)月、12個(gè)月時(shí)AMH水平較初測時(shí)均明顯降低(P0.05),12個(gè)月時(shí)AMH水平較6個(gè)月時(shí)明顯降低(P0.05);12個(gè)月時(shí)LH、FSH水平較初測、6個(gè)月時(shí)均明顯升高(P0.05),E2水平較初測、6個(gè)月時(shí)均明顯降低(P0.05)。正常組中妊娠婦女入組的初測AMH水平明顯高于非妊娠婦女的初測水平(P0.05)。結(jié)論 AMH能較早、準(zhǔn)確地反映高齡女性的卵巢功能變化,評估其自然受孕能力,因此在高齡婦女的生育能力評估中具有高的臨床應(yīng)用價(jià)值。
[Abstract]:Objective to investigate the value of anti-mullerian hormone AMH in the evaluation of ovarian function in elderly women. Methods 100 elderly women who received consultation in the outpatient clinic of Liuzhou Maternal and Child Health Hospital from October 2014 to February 2015 were collected. According to the menstrual state, 32 cases were divided into normal group, 35 cases in peri-menopausal period and 33 cases in premature Ovarian failure (POF) group. The levels of luteinizing Hormoneinizing LHH, Follicle-Stimulating HormoneFSHSH2, Estradiol E2 and Estradiolol E2 were compared. To analyze the changes of FSHE2 and AMH levels in peri-menopausal women at 6 months and 12 months, respectively. Results the serum FSH level in the perimenopausal group was significantly higher than that in the normal group (P 0.05) and that in the control group was significantly higher than that in the normal group. The level of P0.05E2AH in the peri-menopausal group was significantly lower than that in the normal group and the peri-menopausal group. The AMH level in the peri-menopausal group was significantly lower than that in the first measured group at 12 months, the AMH level at 12 months was significantly lower than that at 6 months, and at 12 months after menopause, it was significantly lower than that at 6 months after entering the group, and at 12 months after menopause, the AMH level was significantly lower than that at 6 months. The level of AMH in normal pregnant women was significantly higher than that in non-pregnant women (P 0.05). Conclusion the level of AMH was earlier in normal pregnant women than that in non-pregnant women, and the level of P0. 05% E 2 was significantly higher than that in normal women at 6 months, and the level of P0. 05% E 2 was significantly lower in normal pregnant women than that in non pregnant women at 6 months, and the level of P0. 05 in normal women was significantly higher than that in non pregnant women. It can accurately reflect the changes of the ovarian function of the elderly women and evaluate their natural ability to conceive, so it has high clinical application value in the evaluation of the fertility of the elderly women.
