【摘要】：目的采用經(jīng)會(huì)陰三維超聲探討陰道分娩中不同助產(chǎn)方式對(duì)婦女盆底功能的影響。方法選取經(jīng)陰道分娩的產(chǎn)婦84名,按助產(chǎn)方式的不同分為會(huì)陰完整組32例、會(huì)陰側(cè)切組30例、產(chǎn)鉗助產(chǎn)組22例。分別獲得靜息狀態(tài)、瓦氏動(dòng)作和縮肛動(dòng)作時(shí)盆隔裂孔圖像,比較3組婦女產(chǎn)后盆隔裂孔前后徑和面積的差異。結(jié)果會(huì)陰完整組和會(huì)陰側(cè)切組于靜息狀態(tài)、瓦氏動(dòng)作和縮肛動(dòng)作時(shí)盆隔裂孔的前后徑、面積差異均無統(tǒng)計(jì)學(xué)意義(P均0.05)。會(huì)陰完整組和產(chǎn)鉗助產(chǎn)組在靜息狀態(tài)、瓦氏動(dòng)作和縮肛動(dòng)作時(shí)的盆隔裂孔前后徑、面積,靜息—瓦氏和靜息—縮肛時(shí)盆隔裂孔改變面積差異均有統(tǒng)計(jì)學(xué)意義(P均0.05)。結(jié)論產(chǎn)鉗對(duì)盆底功能的損傷大于自然陰道分娩,陰道分娩中,是否會(huì)陰切開對(duì)盆底功能損傷無明顯差異。
[Abstract]:Objective to study the effects of different midwifery modes on pelvic floor function of women in vaginal delivery by transperineal three-dimensional ultrasound. Methods 84 parturient women who were delivered by vagina were divided into perineum complete group (32 cases) and perineal lateral resection group (30 cases). Twenty-two cases in the forceps assisted labor group were given the images of pelvic septum in resting state, Watt's motion and anus contraction, respectively. The difference of the diameter and area of the postpartum pelvic fissure was compared among the 3 groups. Results the perineum intact group and perineal lateral incision group were in resting state. There was no significant difference in the anteroposterior diameter and area of the pelvic septal hole between the two movements (P < 0.05). The perineum intact group and the midwifery group were in the resting state, the anterior and posterior diameter and area of the pelvic septal hole in the Watts movement and the anus contraction movement, and the area of the pelvic septal hole in the perineum intact group and the labor forceps midwifery group. There were statistically significant differences in the area of pelvic septum between resting and receding sphincter (P < 0.05). Conclusion the damage of forceps to pelvic floor function is greater than that of natural vaginal delivery. There was no significant difference between perineal incision and pelvic floor function injury.
