【摘要】：目的探討固體FTA樣本洗提卡(FTA卡)在宮頸癌篩查的HPV DNA分型檢測中作為儲存與轉(zhuǎn)運待檢樣本載體的臨床應(yīng)用價值。方法以北京大學(xué)深圳醫(yī)院宮頸門診因HPV或細(xì)胞學(xué)檢查陽性行陰道鏡下宮頸活檢的102例患者為研究對象,由婦科醫(yī)師采集宮頸脫落細(xì)胞樣本,分別使用固體FTA卡和細(xì)胞保存液作為樣本儲存和轉(zhuǎn)運的介質(zhì),均采用PCR為基礎(chǔ)的質(zhì)量陣列基質(zhì)輔助激光電離解析飛行時間質(zhì)譜系統(tǒng)(MALDI-TOF)法進(jìn)行高危型HPV分型檢測,比較FTA卡與常規(guī)的液體介質(zhì)作為樣本儲存與轉(zhuǎn)運載體在宮頸癌篩查中的有效性。結(jié)果102例患者中,液體介質(zhì)保存的樣本檢測的HPV感染率為69.6%,FTA卡為66.7%,兩組比較,差異無統(tǒng)計學(xué)意義(χ~2=0.250,P0.05)。兩種保存方法檢測HPV陽性率有很好的一致性(Kappa值=0.932,P0.001)。兩種方法檢測的HPV型別比較,差異無統(tǒng)計學(xué)意義(χ~2=0.250,P0.05),且兩者具有很好的一致性(Kappa=0.930,P0.001)。以病理診斷為金標(biāo)準(zhǔn),FTA卡和液體介質(zhì)保存樣本檢測HPV對于檢出≥CIN2的曲線下面積分別為0.718和0.699(P0.05);檢出≥CIN3的曲線下面積分別為0.695和0.678(P0.05)。結(jié)論 FTA卡和液體介質(zhì)保存宮頸細(xì)胞樣本在HPV分型檢測中具有良好的一致性,對于高級別宮頸病變的診斷效率亦相當(dāng)。由于固體FTA卡體積小、便于轉(zhuǎn)運、污染風(fēng)險小、且簡化了樣本DNA的提取,故將其作為一種新的HPV檢測樣本儲存方法用于宮頸癌篩查具有很高的臨床應(yīng)用價值。
[Abstract]:Objective to investigate the clinical application value of solid FTA sample elution card in HPV DNA typing of cervical cancer screening as a carrier for storing and transporting samples to be tested. Methods HPV or cells were used in cervical clinic of Shenzhen Hospital of Peking University. 102 patients who were positive for cervical biopsy under colposcopy were studied. Samples of cervical exfoliated cells were collected by gynecologists, and solid FTA cards and cell preservatives were used as media for sample storage and transport, respectively. High risk HPV typing was detected by mass array matrix assisted laser ionization time of flight mass spectrometry (MALDI-TOF) based on PCR. To compare the effectiveness of FTA card and conventional liquid media as sample storage and transport carrier in cervical cancer screening. Results among 102 patients, the infection rate of HPV detected by liquid media preserved samples was 66.7%. There was no significant difference between the two methods (蠂 ~ (2 +) 0.250 (P 0.05)). There was a good consistency between the two preservation methods for the positive rate of HPV. The HPV types detected by the two methods were compared.
