【摘要】：目的分析對(duì)比傳統(tǒng)術(shù)式與經(jīng)陰道網(wǎng)片盆底重建術(shù)治療老年重度盆底器官脫垂(POP)的臨床療效。方法選擇2012年1月至2014年1月該院收治的重度POP患者70例,根據(jù)雙盲法將其平均分為置入網(wǎng)片組與傳統(tǒng)術(shù)式組,每組各35例。置入網(wǎng)片組患者采用經(jīng)陰道網(wǎng)片盆底重建術(shù)治療,傳統(tǒng)術(shù)式組采用傳統(tǒng)術(shù)式治療。觀察對(duì)比兩組手術(shù)時(shí)間、出血量、尿管留置時(shí)間、醫(yī)療費(fèi)用及住院時(shí)間。應(yīng)用盆底障礙問(wèn)卷(PFDI-20)及盆底功能影響問(wèn)卷(PFIQ-7)評(píng)價(jià)療效,對(duì)比兩組治愈、復(fù)發(fā)及術(shù)后并發(fā)癥情況。結(jié)果 1兩組手術(shù)時(shí)間、出血量、尿管留置時(shí)間及住院時(shí)間對(duì)比無(wú)明顯差異(P0.05)。置入網(wǎng)片組醫(yī)療費(fèi)用明顯高于傳統(tǒng)術(shù)式組(P0.05)。2兩組術(shù)后6個(gè)月時(shí)PFDI-20、PFIQ-7評(píng)分均低于術(shù)前(P0.05),置入網(wǎng)片組術(shù)后6個(gè)月時(shí)PFDI-20、PFIQ-7評(píng)分顯著低于傳統(tǒng)術(shù)式組(P0.05)。3置入網(wǎng)片組治愈率為100.00%、復(fù)發(fā)率為0.00%,傳統(tǒng)術(shù)式組為85.71%、14.29%,兩組治愈率及復(fù)發(fā)率對(duì)比差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。4傳統(tǒng)術(shù)式組無(wú)明顯并發(fā)癥出現(xiàn),置入網(wǎng)片組出現(xiàn)2例陰道網(wǎng)片暴露。結(jié)論經(jīng)陰道網(wǎng)片盆底重建術(shù)能夠有效改善老年重度POP患者的臨床癥狀和盆底功能,具有治愈率高,術(shù)后患者復(fù)發(fā)率低、生活質(zhì)量較好等特點(diǎn),值得臨床推廣。
[Abstract]:Objective to analyze the treatment of severe pelvic floor prolapse (POP) in elderly patients with pelvic floor reconstruction by transvaginal mesh compared with traditional operation. Methods from January 2012 to January 2014, 70 patients with severe POP were selected. According to the double blind method, the patients in each group were divided into two groups: netting group (35 cases) and traditional operation group (35 cases). The patients in the netting group were treated with transvaginal mesh reconstruction of pelvic floor. The traditional operation group was treated with the traditional operation method. The operation time, blood loss and urinary catheter retention time were observed and compared between the two groups. The curative effect was evaluated by PFDI-20 and PFIQ-7). The curative effect was compared between the two groups. Results 1 the time of operation and the amount of blood loss in the two groups. There was no significant difference in indwelling time and hospitalization time between the two groups (P 0.05). The medical cost in the mesh placement group was significantly higher than that in the traditional operation group at 6 months after operation. The PFIQ-7 score was lower than that before operation (P 0.05). The PFDI-20 in the netting group was 6 months after operation. The PFIQ-7 score was significantly lower than that in the traditional operation group (P 0.05). The cure rate and recurrence rate were 10 0. 00 and 0. 00g respectively in the traditional operation group and 85.71% in the traditional operation group. The difference of cure rate and recurrence rate between the two groups was statistically significant (P 0.05). 4 there were no obvious complications in the traditional operation group. Conclusion Transvaginal reconstruction of pelvic floor can effectively improve the clinical symptoms and pelvic floor function of elderly patients with severe POP, and has a high cure rate and a low recurrence rate. Better quality of life and other characteristics, worthy of clinical promotion.
