【摘要】：目的:探討術(shù)后輔助化療對(duì)IA2-IB1期單純低分化宮頸鱗癌患者預(yù)后的影響。方法:采用回顧性分析方法,收集2003年2月~2014年2月就診于廣西醫(yī)科大學(xué)第一附屬醫(yī)院婦科的76例IA2-IB1期單純低分化高危因素宮頸鱗癌手術(shù)患者的病例資料,根據(jù)術(shù)后輔助化療情況,分為觀察組(手術(shù)+術(shù)后化療組42例)和對(duì)照組(單純手術(shù)組34例),對(duì)比兩組患者預(yù)后生存指標(biāo),并總結(jié)觀察組患者的化療毒副反應(yīng)。結(jié)果:所有患者累計(jì)總生存率、無復(fù)發(fā)生存率、無遠(yuǎn)處轉(zhuǎn)移生存率及無局部復(fù)發(fā)生存率分別為93.9%、94.7%、96.1%、98.7%,其中觀察組患者總生存率、無復(fù)發(fā)生存率均優(yōu)于對(duì)照組(P0.05),且遠(yuǎn)處轉(zhuǎn)移率(0%)低于對(duì)照組(8.8%),而無局部復(fù)發(fā)生存率兩組患者比較差異無顯著性(P0.05)。觀察組所有患者均完成了所有療程化療,化療毒副反應(yīng)主要為胃腸道反應(yīng)23.81%(10/42),骨髓抑制19.04%(8/42),周圍神經(jīng)障礙14.29%(6/42),肝腎功能損害4.76%(2/42),均為可逆性。結(jié)論:針對(duì)單純低分化高危因素的IA2-IB1期宮頸鱗癌患者,術(shù)后輔助化療不僅提高患者總生存率,還降低復(fù)發(fā)率,且化療不良反應(yīng)的可逆性在提高患者的生活質(zhì)量中具有一定優(yōu)勢(shì),是一種可行的治療選擇。
[Abstract]:Objective: to investigate the effect of postoperative adjuvant chemotherapy on prognosis of patients with simple poorly differentiated squamous cell carcinoma of the cervix (IA2-IB1). From February 2003 to February 2014, 76 cases of cervical squamous cell carcinoma (SCC) with low differentiation and high risk factors were collected from gynecological department of the first affiliated Hospital of Guangxi Medical University. According to the postoperative adjuvant chemotherapy, the patients were divided into observation group (42 cases of postoperative chemotherapy group) and control group (34 cases of simple operation group). Results: the cumulative overall survival rate, recurrence free survival rate, distant metastasis free survival rate and local recurrence free survival rate of all patients were 93.9% or 94.7% respectively. The overall survival rate and recurrence free survival rate of the patients in the observation group were better than those in the control group (P 0.05), and the distant metastasis rate was lower than that in the control group (P < 0.01). However, there was no significant difference between the two groups in the survival rate of no local recurrence (P 0.05). All the patients in the observation group completed all the courses of chemotherapy. The side effects of chemotherapy were mainly gastrointestinal reaction 23.81% 10 / 42, bone marrow suppression 19.0410 / 42, peripheral nerve disturbance 14.29 / 42). Liver and kidney function damage 4.76 / 42%, are reversible. Conclusion: simple low differentiation risk factors for IA2-IB1 patients with cervical squamous cell carcinoma. Postoperative adjuvant chemotherapy not only improves the overall survival rate, but also reduces the recurrence rate. The reversibility of adverse reactions of chemotherapy has some advantages in improving the quality of life of patients, and it is a feasible treatment choice.
