【摘要】：目的探討雙管法硬脊膜外鎮(zhèn)痛對胎頭位置異常產(chǎn)婦分娩結(jié)局及分娩方式的影響。方法選取2015年9月-2016年6月該院產(chǎn)科進行陰道試產(chǎn)的127例胎頭位置異常產(chǎn)婦進前瞻性行研究,采用隨機數(shù)字表法分為單管組、雙管組,63例采用單管法硬脊膜外鎮(zhèn)痛(單管組)、64例采用雙管法硬脊膜外鎮(zhèn)痛(雙管組),對比兩組的分娩結(jié)局及分娩方式。結(jié)果雙管組的陰道分娩率64.06%高于單管組的46.04%(P0.05);雙管組的剖宮產(chǎn)率23.44%和縮宮素使用率60.94%低于單管組的41.27%和90.48%,差異均有統(tǒng)計學(xué)意義(P0.05);兩組陰道分娩產(chǎn)婦分娩鎮(zhèn)痛前的宮口大小、第三產(chǎn)程差異無統(tǒng)計學(xué)意義(P0.05);雙管組的第一、第二產(chǎn)程均低于單管組,差異有統(tǒng)計學(xué)意義(P0.05);兩組產(chǎn)婦的產(chǎn)后出血量、尿潴留發(fā)生率差異無統(tǒng)計學(xué)意義(P0.05)。兩組產(chǎn)婦的兩組新生兒體重、第1及5分鐘的Apgar評分差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論雙管法硬脊膜外鎮(zhèn)痛有利于提高胎頭位置異常產(chǎn)婦陰道分娩率,同時縮短產(chǎn)程。
[Abstract]:Objective to investigate the effect of double tube epidural analgesia on the delivery outcome and delivery mode of women with abnormal position of fetal head. Methods 127 cases of vaginal labor were performed in our hospital from September 2015 to June 2016. Prospective study of abnormal cephalic position of parturient. 63 cases of double tube group were treated with single tube epidural analgesia. 64 cases of single tube group were treated with double tube method for epidural analgesia (double tube group). Results the vaginal delivery rate in the double tube group was 64.06% higher than that in the single tube group (46.04). The cesarean section rate (23.44%) and oxytocin utilization rate (60.94%) in the double tube group were lower than those in the single tube group (41.27% and 90.48), and the difference was statistically significant (P 0.05). There was no significant difference in the size of uterine orifice before labor analgesia between the two groups, and there was no significant difference in the third stage of labor between the two groups (P 0.05). The first and second stages of labor in the double tube group were lower than those in the single tube group, and the difference was statistically significant (P 0.05). There was no significant difference in the incidence of postpartum hemorrhage and urinary retention between the two groups (P 0.05). There was no significant difference in Apgar score between the first and fifth minute. Conclusion the double tube epidural analgesia is helpful to improve the vaginal delivery rate and shorten the labor process of women with abnormal position of fetal head.
