【摘要】：目的:探討PDCA循環(huán)管理法干預剖宮產(chǎn)圍術期抗菌藥物預防使用的效果,促進抗菌藥物的合理應用。方法:運用PDCA循環(huán)理論進行計劃、實施、確認和處置,比較評價實施PDCA循環(huán)管理前后剖宮產(chǎn)圍術期抗菌藥物預防使用的合理性。結果:經(jīng)過2個PDCA循環(huán)干預階段后,無高危因素剖宮產(chǎn)手術抗菌藥物選藥合理性由54.25%上升至94.19%,持續(xù)預防給藥時間由全部超24 h,個別病歷甚至超48 h降至全部控制在24 h內(nèi);存在高危因素預防使用抗菌藥物持續(xù)時間超過48 h病歷由10.30%降至全部控制在48 h內(nèi)。結論:PDCA循環(huán)管理能有效促進剖宮產(chǎn)圍術期抗菌藥物的合理應用,且可循序推進,符合醫(yī)療質(zhì)量持續(xù)改進的要求,值得在抗菌藥物臨床應用管理中推廣。
[Abstract]:Objective: to investigate the effect of PDCA circulatory management on the prevention and use of antibiotics in perioperative period of cesarean section, and to promote the rational application of antibiotics. Methods: the theory of PDCA circulation was used to plan and implement the antimicrobial drugs. To confirm and dispose, compare and evaluate the rationality of prophylactic use of antibiotics in perioperative period of cesarean section before and after PDCA circulation management. Results: after two PDCA cycle intervention stages. The rationality of antibiotics selection in cesarean section without high risk factors increased from 54.25% to 94.19, and the duration of continuous prophylactic administration was more than 24 hours. Individual medical records even exceeded 48 hours and all of them were controlled within 24 hours. The duration of prophylactic use of antimicrobial agents decreased from 10.30% to 48 hours. PDCA circulation management can effectively promote the rational use of antibiotics in perioperative period of cesarean section. It can be promoted step by step and meets the requirement of continuous improvement of medical quality. It is worth popularizing in clinical application management of antimicrobial agents.
