【摘要】：目的探討不同營養(yǎng)支持方案在極低出生體重兒(VLBWI)中的應用效果。方法選取2014年3月-2015年12月該院收治的行早期積極營養(yǎng)支持方案的45例VLBWI歸為早期組,然后選取2013年1月-2014年2月收治的行傳統(tǒng)滯后靜脈營養(yǎng)支持方案的40例VLBWI為滯后組。比較兩組的臨床資料、并發(fā)癥發(fā)生狀況及實驗室檢查結(jié)果。結(jié)果體重的下降幅度、恢復出生體重的時間、體重達到2 000 g的日齡、靜脈營養(yǎng)支持的持續(xù)天數(shù)、總住院時間等均早期組滯后組,恢復出生體重后的體重平均增長速度、出生后第7天的熱卡總量等早期組滯后組,差異有統(tǒng)計學意義(P0.05)。所有患兒均未出現(xiàn)氮質(zhì)血癥、靜脈炎、血栓形成等并發(fā)癥,早期組的喂養(yǎng)不耐受(FI)、宮外生長遲緩(EUGR)等并發(fā)癥發(fā)生率低于滯后組,差異有統(tǒng)計學意義(P0.05)。兩組的高膽紅素血癥、膽汁淤積、壞死性小腸結(jié)腸炎(NEC)等并發(fā)癥發(fā)生率比較,差異無統(tǒng)計學意義(P0.05)。兩組的血總膽紅素(TB)、結(jié)合膽紅素(CB)、血肌酐(Scr)、尿素氮(BUN)、丙氨酸氨基轉(zhuǎn)移酶(ALT)等指標比較,差異無統(tǒng)計學意義(P0.05)。結(jié)論對VLBWI而言,早期給予積極營養(yǎng)支持能促進患兒的生長發(fā)育,降低FI和EUGR的發(fā)生風險,安全可行,值得臨床推廣應用。
[Abstract]:Objective to investigate the effect of different nutritional support schemes on (VLBWI) in very low birth weight infants. Methods from March 2014 to December 2015, 45 cases of VLBWI treated with early active nutrition support program were divided into early group, and then 40 cases of VLBWI treated with traditional delayed intravenous nutrition support program from January 2013 to February 2014 were selected as lag group. The clinical data, complications and laboratory results of the two groups were compared. Results the rate of weight loss, the time to recover birth weight, the age of 2 000 g body weight, the duration of intravenous nutrition support and the total hospitalization time were all in the early lag group, the average weight gain rate after recovery of birth weight and the total amount of heat card on the 7th day after birth, the difference was statistically significant (P 0.05). There were no complications such as azotemia, phlebitis and thrombosis in all children. The incidence of complications such as (FI), extrauterine growth retardation (EUGR) in the early group was lower than that in the lag group, the difference was statistically significant (P 0.05). There was no significant difference in the incidence of hypercholinemia, cholestasis and (NEC) between the two groups (P 0.05). There was no significant difference between the two groups (P 0.05). There was no significant difference between the two groups (P 0.05). Conclusion for VLBWI, early active nutritional support can promote the growth and development of children, reduce the risk of FI and EUGR, safe and feasible, and is worthy of clinical application.
