依托咪酯用量對左向右分流先天性心臟病患兒血流動力學及腦電雙頻指數的影響

【摘要】:目的·分析左向右分流先天性心臟病患兒連續(xù)泵注依托咪酯鎮(zhèn)靜時劑量與腦電雙頻指數(BIS)的相關性。方法·24例左向右分流先天性心臟病患兒全身麻醉誘導后停止吸入七氟醚,使BIS值逐漸恢復至80;經外周靜脈按0.3 mg/(kg·min)劑量持續(xù)泵注依托咪酯,待BIS值為40時停止泵入;記錄期間BIS為80、70、60、50和40時心率(HR)、動脈壓(ABP)和中心靜脈壓(CVP)的變化及依托咪酯總的用量,分析左向右分流先天性心臟病患兒連續(xù)泵注依托咪酯鎮(zhèn)靜時劑量與BIS的相關性。結果·依托咪酯持續(xù)泵注至BIS值達到40時,患兒血流動力學變化不明顯,依托咪酯與BIS值呈負相關(r=-0.769,P0.01);2歲以下先天性心臟病患兒依托咪酯平均劑量高于2歲以上先天性心臟病患兒(P=0.034)。結論·依托咪酯對左向右分流先天性心臟病患兒血流動力學的影響較小,泵注劑量與BIS值呈負相關;左向右分流先天性心臟病患兒BIS與年齡可能有一定關系。
[Abstract]:Objective to analyze the correlation between the dose of etomidate sedation and EEG bispectral index (BIS) in children with left-to-right shunt congenital heart disease. Methods Twenty-four children with left-to-right shunt congenital heart disease stopped inhaling sevoflurane after general anesthesia induction, and the BIS value was gradually restored to 80. Etomidate was continuously injected into the peripheral vein at a dose of 0.3 mg/ (kg 路min), and stopped pumping when the BIS value was 40. During the recording period, the changes of heart rate (HR), arterial pressure (ABP) and central venous pressure (CVP) and the total dosage of etomidate were 80, 70, 60, 50 and 40, respectively.
