血管迷走性暈厥患兒基于24h尿鈉水平臨床干預(yù)的研究

【摘要】:目的:探討血管迷走性暈厥(vasovagal syncope,VVS)患兒基于24h尿鈉水平臨床干預(yù)后尿鈉及血流動(dòng)力學(xué)參數(shù)的變化,評(píng)價(jià)口服補(bǔ)液鹽及美托洛爾對(duì)低尿鈉VVS患兒的臨床療效。方法:選取2014年6月至2016年3月在蘭州大學(xué)第二醫(yī)院兒童醫(yī)院門(mén)診就診或住院的臨床有反復(fù)暈厥或暈厥先兆表現(xiàn)(暈厥頻次≥2次)的兒童,經(jīng)詳細(xì)詢(xún)問(wèn)病史、體格檢查及輔助檢查,排除發(fā)熱、感染及器質(zhì)性疾病所致暈厥,符合《兒童暈厥診斷指南》標(biāo)準(zhǔn),征得患兒家屬書(shū)面知情同意后,經(jīng)直立傾斜試驗(yàn)(head-up tilt testing,HUTT)確診為VVS的患兒共120例,根據(jù)所測(cè)24h尿鈉選取124mmol/24h的患兒79例作為研究對(duì)象,按數(shù)據(jù)表法隨機(jī)分為溫開(kāi)水組(n=15例)、口服補(bǔ)液鹽組(n=21例)、美托洛爾組(n=21例)、口服補(bǔ)液鹽與美托洛爾聯(lián)合治療組(n=22例)。比較低尿鈉患兒干預(yù)3月后HUTT的結(jié)果,分析干預(yù)后各組24h尿鈉、血清鈉水平及血流動(dòng)力學(xué)參數(shù)變化,評(píng)價(jià)不同干預(yù)方案臨床療效。相關(guān)數(shù)據(jù)均采用SPSS 19.0統(tǒng)計(jì)學(xué)軟件進(jìn)行數(shù)據(jù)處理、分析。結(jié)果:(1)口服補(bǔ)液鹽組和聯(lián)合治療組治療后24h尿鈉水平與治療前比較顯著升高,聯(lián)合治療組尿鈉升高幅度高于口服補(bǔ)液鹽組(P0.05),治療后血清鈉水平均無(wú)明顯變化(P0.05)。聯(lián)合治療組和美托洛爾組治療后,直立傾斜即刻與基礎(chǔ)心率的差值較治療前縮小(P0.05)。(2)溫開(kāi)水組、口服補(bǔ)液鹽組、美托洛爾組、聯(lián)合治療組治療3月后的有效率分別為26.67%(4/15例)、66.67%(14/21例)、71.43%(15/21例)、95.45%(21/22例)。HUTT轉(zhuǎn)陰率分別為6.67%(1/15例)、43.86%(9/21例)、52.38%(11/21例)、86.36%(19/22例)。聯(lián)合治療組治療的有效率和HUTT轉(zhuǎn)陰率優(yōu)于其他三組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。(3)VVS血管抑制型、心臟抑制型和混合型之間24h尿鈉、血清鈉血流動(dòng)力學(xué)參數(shù)及各組療效比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:低尿鈉VVS患兒有明顯血流動(dòng)力學(xué)改變,臨床干預(yù)后得到改善;口服補(bǔ)液鹽與美托洛爾聯(lián)合治療的臨床療效及HUTT轉(zhuǎn)陰率優(yōu)于單一治療,是VVS臨床干預(yù)安全有效的方法;口服補(bǔ)液鹽聯(lián)合美托洛爾對(duì)VVS三種血流動(dòng)力學(xué)反應(yīng)類(lèi)型療效一致。
