【摘要】：小兒厭食癥是兒科臨床常見病、多發(fā)病,據(jù)報(bào)道嬰兒和學(xué)齡前兒童發(fā)病率約為12%-34%,小兒厭食癥如反復(fù)發(fā)作、遷延不愈,可影響兒童正常生長發(fā)育,降低免疫力,引起維生素D缺乏性佝僂病、缺鐵性貧血、反復(fù)呼吸道感染等疾病,臨床值得重視。馮氏捏積療法是全國著名老中醫(yī)馮全福教授在馮氏祖?zhèn)魉拇�、歷時(shí)150年的臨床經(jīng)驗(yàn)上總結(jié)歸納出的安全、無創(chuàng)傷、成熟的中醫(yī)特色療法,治療疾病簡、便、驗(yàn)、廉,在北京地區(qū)乃至全國都有較大的影響和良好的口碑。馮氏捏積療法治療小兒厭食癥臨床療效良好,但多年來缺乏完整的技術(shù)診療規(guī)范及技術(shù)操作規(guī)范,限制了該療法的推廣應(yīng)用。本研究采用科學(xué)的方法,對(duì)馮氏捏積療法的規(guī)范化操作進(jìn)行初步研究,制定了馮氏捏積療法技術(shù)操作規(guī)范文本,并應(yīng)用規(guī)范化馮氏捏積療法治療小兒厭食癥129例,臨床療效確切,為該療法的推廣應(yīng)用提供一定的科學(xué)證據(jù)。 本論文包括文獻(xiàn)綜述和臨床研究兩部分內(nèi)容。 文獻(xiàn)綜述部分通過閱讀近5年中醫(yī)治療小兒厭食癥及捏積療法臨床應(yīng)用和機(jī)理研究的文獻(xiàn)報(bào)道,對(duì)小兒厭食癥的病因病機(jī)、治法治則、內(nèi)治法、外治法進(jìn)行總結(jié)；對(duì)捏積療法的中醫(yī)治療疾病機(jī)理及現(xiàn)代研究進(jìn)展進(jìn)行回顧和總結(jié)。 臨床研究部分包括馮氏捏積療法規(guī)范化操作的研究及馮氏捏積療法規(guī)范化治療小兒厭食癥129例臨床療效研究。 目的：觀察馮氏捏積療法規(guī)范化治療小兒厭食癥的臨床療效。 方法：應(yīng)用馮氏捏積療法規(guī)范化治療129例小兒厭食癥患者,觀察患兒治療后臨床癥狀、中醫(yī)癥狀量化積分、D-木糖吸收排泄率評(píng)價(jià)該療法對(duì)小兒厭食癥的臨床治療效果。 結(jié)果：馮氏捏積療法規(guī)范化治療小兒厭食癥129例,總有效率90.1%。治療前后中醫(yī)癥狀量化積分分別是13.35±5.58、6.62±1.62,積分差異有顯著統(tǒng)計(jì)學(xué)意義(P0.01)。治療前后D-木糖吸收排泄率分別是16.44±6.56,18.19±5.56,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。 結(jié)論：馮氏捏積療法治療小兒厭食癥的臨床療效確切,能顯著改善小兒厭食癥患者中醫(yī)臨床癥狀,提高小兒厭食癥患者D-木糖吸收排泄率。
[Abstract]:Infantile anorexia is a common and frequently occurring disease in pediatrics. It is reported that the incidence of anorexia in infants and pre-school children is about 12% / 34%. If anorexia occurs repeatedly and does not heal, it can affect the normal growth and development of children and reduce immunity. Lead to vitamin D deficiency rickets, iron deficiency anemia, recurrent respiratory infection and other diseases, clinical attention. Feng's kneading therapy has a good clinical effect on infantile anorexia, but the lack of complete technical diagnosis and treatment and technical practice for many years has limited the popularization and application of this therapy. In this study, a scientific method was adopted to study the standardized operation of Feng's kneading therapy, and the text of the technical practice of Feng's kneading therapy was formulated, and 129 cases of infantile anorexia were treated with standardized Feng's kneading therapy. The clinical curative effect is definite, which provides certain scientific evidence for the popularization and application of this therapy. This paper includes two parts: literature review and clinical research. The part of literature review summarizes the etiology, pathogenesis, internal and external treatment of anorexia in children by reading the literature reports on the clinical application and mechanism of traditional Chinese medicine in treating anorexia in recent 5 years. This paper reviews and summarizes the mechanism and modern research progress of kneading therapy in traditional Chinese medicine (TCM). The clinical research part includes the study of standardized operation of Feng's kneading therapy and the clinical curative effect study of 129 cases of infantile anorexia treated by Feng's kneading therapy. Objective: to observe the clinical effect of Feng's kneading therapy on infantile anorexia. Methods: 129 cases of infantile anorexia were treated with Feng's kneading therapy. The clinical symptoms, the quantitative integral of TCM symptoms, and the absorption and excretion rate of D-xylose were observed after treatment to evaluate the therapeutic effect of this therapy on anorexia in children. Results: 129 cases of infantile anorexia were treated by Feng's kneading therapy, the total effective rate was 90.1%. Before and after treatment, the quantitative scores of TCM symptoms were 13.35 鹵5.58 and 6.62 鹵1.62, respectively. There was significant difference in the scores between before and after treatment (P0.01). The absorption and excretion rate of D-xylose was 16.44 鹵6.56 and 18.19 鹵5.56 respectively before and after treatment (P0.05). Conclusion: Feng's kneading therapy is effective in the treatment of infantile anorexia. It can significantly improve the clinical symptoms of anorexia and increase the absorption and excretion rate of D-xylose in children with anorexia.
