【摘要】：目的:探討麻杏石甘湯聯(lián)合阿奇霉素對小兒支原體肺炎(MPP)患者抗炎、促炎因子及免疫功能的影響。方法:選取河南中醫(yī)藥大學第二附屬醫(yī)院2014年11月—2016年4月收治的165例MPP患兒,按隨機數(shù)字表法分為治療組(83例)和對照組(82例)。對照組MPP患兒給予阿奇霉素序貫療法進行治療,治療組聯(lián)合麻杏石甘湯進行治療。結(jié)果:治療組患者總有效率為93.98%,對照組為81.71%,治療組優(yōu)于對照組(P0.05);與治療前比較,治療后,兩組患兒血清中白細胞介素-2(IL-2)水平明顯升高,且治療組明顯高于對照組(P0.05),兩組患兒白細胞介素-4(IL-4),白細胞介素-10(IL~(-1)0)水平明顯降低,且治療組明顯低于對照組(P0.05);與治療前比較,治療后兩組患兒血清中白細胞介素-6(IL-6),腫瘤細胞因子-α(TNF-α)與干擾素-γ(IFN-γ)水平明顯降低,且治療組低于對照組(P0.05);與治療前比較,治療后,兩組患兒血清中CD3+,CD4+以及紅細胞C3b受體(RBC-C3bR)水平明顯升高,且治療組明顯高于對照組(P0.05),CD8+與紅細胞免疫復合物(RBC-ICR)水平明顯降低,且治療組明顯低于對照組(P0.05);與治療前比較,治療后兩組中醫(yī)證候積分均有明顯改善(P0.05),與對照組相比,治療組中醫(yī)證候積分低于對照組(P0.05),兩組不良反應比較,差異無統(tǒng)計學意義。結(jié)論:麻杏石甘湯聯(lián)合阿奇霉素治療MPP患兒療效顯著,可能與其升高抗炎因子,降低促炎因子水平,改善患兒免疫功能有關(guān)。
[Abstract]:Objective: to investigate the effect of maxing Shigan decoction combined with azithromycin on anti-inflammatory, proinflammatory factor and immune function in children with mycoplasma pneumonia (MPP). Methods: 165 children with MPP were randomly divided into treatment group (n = 83) and control group (n = 82) from November 2014 to April 2016 in the second affiliated Hospital of Henan University of traditional Chinese Medicine. The control group (MPP) was treated with azithromycin sequential therapy, and the treatment group with Maxingshigan decoction. Results: the total effective rate was 93.98 in the treatment group and 81.71 in the control group. The treatment group was superior to the control group (P0.05). Compared with those before treatment, the serum levels of interleukin-2 (IL-2) in the two groups were significantly higher than those in the control group (P0.05). The levels of interleukin-4 (IL-4) in the two groups were significantly higher than those in the control group (P0.05). The level of interleukin-10 (IL~ (-1) 0) was significantly lower in the treatment group than in the control group (P0.05). The levels of serum interleukin-6 (IL-6), tumor cytokine 偽 (TNF- 偽) and interferon- 緯 (IFN- 緯) in the two groups were significantly lower than those in the control group (P0.05). After treatment, the levels of serum CD3, CD4 and erythrocyte C3b receptor (RBC-C3bR) in the two groups were significantly higher than those in the control group (P0.05). The level of CD8 and erythrocyte immune complex (RBC-ICR) was significantly lower in the treatment group than in the control group (P0.05). Compared with the treatment before treatment, the two groups after the treatment of TCM syndrome score improved significantly (P0.05), compared with the control group, the treatment group TCM syndrome score was lower than the control group (P0.05), the two groups of adverse reactions, the difference was not statistically significant. Conclusion: the curative effect of maxing Shigan decoction combined with azithromycin on MPP children may be related to the increase of anti-inflammatory factor, the decrease of proinflammatory factor level and the improvement of immune function in children with MPP.
