【摘要】：目的 探討窒息新生兒發(fā)生多器官損害的影響因素及臨床結(jié)局。方法 選取2007年10月至2016年4月海南省農(nóng)墾總醫(yī)院收治的窒息新生兒115例,依據(jù)多器官損害發(fā)生情況分為多器官損害組(61例)和無多器官損害組(54例),對(duì)入組患兒的臨床資料進(jìn)行單因素和多因素Logistic回歸分析,比較兩組患兒的臨床結(jié)局。結(jié)果 是否存在宮內(nèi)窘迫、羊水污染、機(jī)械通氣、不同入院時(shí)間、入院pH值、Apgar評(píng)分、窒息程度與窒息新生兒的多器官損害發(fā)生率有關(guān),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。宮內(nèi)窘迫、羊水污染、Apgar評(píng)分小于或等于6分、重度窒息是影響窒息新生兒發(fā)生多器官損害的獨(dú)立危險(xiǎn)因素(P0.05)。多器官損害組61例,治愈36例(59.02%),死亡14例(22.95%);無多器官損害組54例,治愈50例(92.59%),死亡2例(3.70%),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論 窒息新生兒發(fā)生多器官損害的風(fēng)險(xiǎn)較高,主要由宮內(nèi)窘迫、羊水污染、Apgar評(píng)分偏低和重度窒息引起,患兒臨床結(jié)局不佳。
[Abstract]:Objective to investigate the influencing factors and clinical outcome of multiple organ damage in asphyxia neonates. Methods from October 2007 to April 2016, 115 cases of asphyxia neonates admitted in Hainan Agricultural Reclamation General Hospital were divided into multiple organ damage group (61 cases) and no multiple organ damage group (54 cases) according to the occurrence of multiple organ damage. Univariate and multivariate Logistic regression analysis were performed to compare the clinical outcome of the two groups. Results Intrauterine distress, amniotic fluid contamination, mechanical ventilation, different admission time, admission pH value, Apgar score and degree of asphyxia were related to the incidence of multiple organ damage in asphyxia neonates (P0.05). Intrauterine distress amniotic fluid contamination Apgar score less than or equal to 6 severe asphyxia is an independent risk factor affecting multiple organ damage in asphyxia neonates (P0.05). Among 61 cases with multiple organ damage, 36 cases (59.02%) were cured and 14 cases (22.95%) died, while in 54 cases without multiple organ damage, 50 cases (92.59%) were cured and 2 cases (3.70%) died. The difference was statistically significant (P0.05). Conclusion the risk of multiple organ damage in asphyxia neonates is higher, mainly caused by intrauterine distress, amniotic fluid contamination, low Apgar score and severe asphyxia.
