【摘要】：目的比較單孔腹腔鏡與傳統(tǒng)開放手術(shù)治療小兒腹股溝嵌頓疝的臨床效果。方法選取我院2014年12月~2015年12月44例小兒腹股溝嵌頓疝,分成腔鏡組和開放組,每組22例患兒,比較2組患兒手術(shù)時(shí)間、術(shù)中出血量、術(shù)后排氣時(shí)間、術(shù)后住院時(shí)間及切口美觀滿意度評(píng)分。結(jié)果 44例手術(shù)順利完成。與開放組相比,腔鏡組手術(shù)時(shí)間明顯縮短[(17.8±4.7)min vs.(30.6±8.2)min,t=-6.352,P=0.000],出血量明顯減少[(4.8±1.3)ml vs.(10.6±3.2)ml,t=-7.876,P=0.000],術(shù)后排氣時(shí)間明顯縮短[(20.3±5.6)h vs.(26.4±7.9)h,t=-2.955,P=0.005],術(shù)后住院時(shí)間明顯縮短[(2.8±0.7)d vs.(4.2±0.9)d,t=-5.759,P=0.000],切口美觀滿意度評(píng)分明顯升高[(4.1±0.9)分vs.(3.0±0.8)分,t=4.285,P=0.000]。44例隨訪(10.6±4.3)月,全部患兒無復(fù)發(fā)及睪丸或卵巢萎縮。結(jié)論單孔腹腔鏡手術(shù)治療小兒腹股溝嵌頓疝安全可行,手術(shù)時(shí)間短,創(chuàng)傷小,恢復(fù)快,并能達(dá)到更好的美容效果,值得臨床推廣。
[Abstract]:Objective to compare the clinical effect of single-hole laparoscopy and traditional open operation in the treatment of infantile inguinal incarcerated hernia. Methods from December 2014 to December 2015, 44 children with incarcerated inguinal hernia were divided into two groups: endoscopic group (n = 22) and open group (n = 22). Postoperative hospitalization time and incision aesthetic satisfaction score. Results 44 cases were successfully completed. Compared with the open group, the operative time of the endoscopic group [(17.8 鹵4.7) min vs. (30.6 鹵8.2) min,t=-6.352,P=0.000], the volume of bleeding significantly decreased [(4.8 鹵1.3) ml vs. (10.6 鹵3.2) ml,t=-7.876,P=0.000], the postoperative exhaust time was significantly shortened [(20.3 鹵5.6) h vs. (26.4 鹵7.9) hti-2.955P0.005], the postoperative hospitalization time was significantly shortened [(2.8 鹵0.7) d vs. (4.2 鹵0.9) dt-5.759P0.000], the aesthetic satisfaction score of the incision was significantly increased [(4.1 鹵0.9) score vs. (3.0 鹵0.8), t 4.285P0. 000] 44 cases were followed up (10.6 鹵4.3) months. No recurrence and atrophy of testis or ovaries were found in all children. Conclusion it is safe and feasible to treat infantile inguinal incarcerated hernia by single hole laparoscopic surgery, with short operation time, less trauma, quick recovery and better cosmetic effect, which is worth popularizing in clinic.
