【摘要】：目的探討高效抗逆轉(zhuǎn)錄病毒治療(HAART)不同治療方案對(duì)人類免疫缺陷病毒(HIV)合并丙型肝炎病毒(HCV)感染患兒肝功能、HIV載量、CD4~+T細(xì)胞的影響。方法 40例患者分為齊多夫定(AZT)+拉米夫定(3TC)+奈韋拉平(NVP)聯(lián)合組13例(A組),AZT+3TC+依非韋倫(EFV)聯(lián)合組16例(B組),司他夫定(D4T)+3TC+EFV治療組5例(C組),D4T+3TC+NVP治療組6例(D組)。檢測(cè)比較4組治療前后肝功能、HIV載量、CD4~+T細(xì)胞。結(jié)果 HAART治療前肝功能異常8例(20%),均為1級(jí)肝損害。4組治療后A、B組ALT、AST上升(P0.01),C組治療后AST上升(P0.01)。HAART治療后各組CD4~+T細(xì)胞明顯升高(P0.01),HIV-RNA病毒載量明顯減低(P0.01)。治療前后HCV-RNA載量變化差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論 HIV合并HCV感染患兒HAART不同治療方案可增加肝損害,但對(duì)HAART治療效果影響有限。
[Abstract]:Objective to investigate the effects of different antiretrovirals on liver function, HIV load and CD4~ T cells in children with hepatitis C virus (HCV) infection and (HIV) infection with human immunodeficiency virus (HIV). Methods Forty patients were divided into three groups: group A (group A, n = 16), group B (n = 16), group D _ 4T (n = 5) treated with 3TC EFV, group D _ 4T, group D _ 4T (n = 6). Liver function, HIV load and CD4~ T cell were measured before and after treatment. Results there were 8 cases (20%) with abnormal liver function before HAART treatment, all of which were grade 1 liver damage. After treatment, ALT,AST in group A and B was increased in group A (P 0.01), C group) (AST increased significantly after treatment with P0.01). HAART (P0.01). HAART), and the viral load of HIV-RNA was significantly decreased (P0.01). There was no significant difference in HCV-RNA load before and after treatment (P0.05). Conclusion different treatment regimens of HAART can increase liver damage in children with HIV complicated with HCV infection, but the effect on HAART is limited.
