【摘要】：目的:基于中醫(yī)傳承輔助平臺,分析導(dǎo)師潘月麗教授治療小兒原發(fā)性遺尿的用藥及組方特點(diǎn),總結(jié)其用藥經(jīng)驗(yàn)。方法:通過收集2012年12月至2016年12月山東中醫(yī)藥大學(xué)附屬醫(yī)院兒科潘月麗教授治療小兒原發(fā)性遺尿的341份門診處方,采用雙人雙錄入的方法逐一錄入“中醫(yī)傳承輔助平臺(V2.5)”,通過平臺“數(shù)據(jù)分析”模塊中“組方分析”對錄入方劑進(jìn)行分析,得出藥物頻率、藥物關(guān)聯(lián)系數(shù)、新方等數(shù)據(jù),從而分析用藥規(guī)律,總結(jié)導(dǎo)師治療經(jīng)驗(yàn)。結(jié)果:(1)常用藥物前20位依次為:甘草、桑螵蛸、益智仁、炙麻黃、石菖蒲、菟絲子、炒白術(shù)、黃芪、柴胡、補(bǔ)骨脂、黨參、覆盆子、烏藥、山藥、遠(yuǎn)志、五味子、雞內(nèi)金、金櫻子、茯苓、升麻。其中補(bǔ)虛藥應(yīng)用最廣泛,占藥物總頻次的38.4%,其次是收澀藥(15.5%)、解表藥(11.9%)、清熱藥(6.2%)、理氣藥(6.0%)、開竅藥(4.6%)等。(2)所用藥物藥性以溫、平為主,累積82.5%;涼、熱性藥使用較少,累積2.2%。藥味以甘、辛、苦類藥物最常用,累積86.5%;酸、咸、澀、淡使用較少,累積13.5%。所用藥物多歸脾、腎、肺、肝、胃、心等經(jīng),累積86.5%;而入膀胱、大腸、膽、小腸、三焦、心包經(jīng)藥物較少,累積13.5%。(3)共得到常用藥物組合27條;可聚類新處方的核心組合6對,新處方6首。結(jié)論:導(dǎo)師治療小兒原發(fā)性遺尿用藥以補(bǔ)虛、收澀為主,強(qiáng)調(diào)“虛”是本病重要的致病因素,認(rèn)為肺脾腎三臟不足,膀胱氣化功能失司是本病的基本病機(jī),而肺失宣肅、心腦閉郁亦與本病相關(guān);治療中標(biāo)本兼顧,并注重宣肺、開竅之品的應(yīng)用。
[Abstract]:Aim: to analyze the characteristics of medication and prescription for primary enuresis in children treated by Professor Pan Yueli, a tutor, based on the platform of traditional Chinese Medicine (TCM) inheritance. Methods: from December 2012 to December 2016, a total of 341 outpatient prescriptions of Pediatrics Professor Pan Yueli, affiliated Hospital of Shandong University of traditional Chinese Medicine, for the treatment of primary enuresis in children were collected. Through the analysis of the input prescription in the "data analysis" module of the platform, the data such as drug frequency, drug correlation coefficient and new prescription are obtained, so as to analyze the law of drug use and sum up the experience of tutor in treatment. Results: (1) the top 20 commonly used drugs were: licorice, odes, Acorus calamus, Cuscuta, Atractylodes macrocephala, Radix Astragali, Radix Bupleurum, psoralen, Codonopsis pilosula, Raspberry, Black Poison, yam, Radix Polygala, Schisandra chinensis, Chicken Internal Gold. Golden cherry, Poria cocos, cohosh. Among them, tonifying and deficiency medicine is the most widely used, accounting for 38.4% of the total frequency of the drug, followed by astringent medicine (15.5%), antitabular medicine (11.9%), heat-clearing medicine (6.2%), Qi regulating medicine (6.0%), prescribing medicine (4.6%), etc. Gan, Xin, bitter medicine is the most commonly used medicine, accumulated 86.5; acid, salty, astringent, light use less, cumulative 13.5. The drugs used were mostly returned to spleen, kidney, lung, liver, stomach, heart and so on, and accumulated 86.5; and entered the bladder, large intestine, bile, small intestine, tri-coke, pericardial meridian less, accumulated 13.5. (3) A total of 27 commonly used drug combinations were obtained, and 6 pairs of core combinations of new prescriptions for clustering were obtained. 6 new prescriptions. Conclusion: the primary enuresis of children treated by tutor is mainly caused by deficiency and astringency. It is emphasized that deficiency is an important pathogenic factor of this disease. It is considered that deficiency of lung, spleen and kidney, loss of bladder gasification function is the basic pathogenesis of this disease, and that of lung loss is Xuansu. Heart-brain stagnation is also associated with this disease; the treatment of both specimens, and focus on the application of lung, orifices.
