【摘要】：目的研究改良心形吻合法治療不同類型小兒先天性巨結(jié)腸的臨床效果。方法選取小兒先天性巨結(jié)腸患者48例,采用隨機數(shù)字法分為對照組和觀察組,每組各24例。對照組患兒行Swenson術(shù)治療,觀察組患兒行改良心形吻合法。比較兩組患兒的各手術(shù)效果指標及小腸結(jié)腸炎、吻合口瘺等并發(fā)癥發(fā)生率,并比較兩組患兒術(shù)后排便功能評分。結(jié)果觀察組的術(shù)中出血量、術(shù)后禁食時間、住院時間及手術(shù)時間均明顯低于對照組(P0.01);觀察組的總并發(fā)癥發(fā)生率為8.33%,對照組的總并發(fā)癥發(fā)生率為33.33%,觀察組明顯低于對照組(X~2=4.55,P=0.03);觀察組術(shù)后的排便功能優(yōu)良率明顯高于對照組(P0.01)。結(jié)論改良心形吻合法治療不同類型小兒先天性巨結(jié)腸的效果顯著,手術(shù)安全性較高,且術(shù)后并發(fā)癥低,能有效促進生活質(zhì)量和排便功能恢復(fù),值得在臨床推廣。
[Abstract]:Objective to study the clinical effect of modified cardiac anastomosis in different types of children with Hirschsprung's disease. Methods 48 children with Hirschsprung's disease were randomly divided into control group and observation group with 24 cases in each group. The patients in the control group were treated with Swenson, and the children in the observation group were treated with modified cardiac anastomosis. The operative outcome indexes and complications such as enterocolitis and anastomotic fistula were compared between the two groups and the postoperative defecation function scores were compared between the two groups. Results the amount of intraoperative bleeding and fasting time after operation were observed in the observation group. The duration of hospitalization and operation was significantly lower than that of the control group (P0.01), the total complication rate of the observation group was 8.33 and the total complication rate of the control group was 33.333.It was obviously lower in the observation group than in the control group (Xan24.55 P0.03), and the excellent and good rate of defecation function in the observation group was significantly higher than that in the control group (P0.01). Conclusion the modified cardiostomy is effective in the treatment of different types of children with Hirschsprung's disease. The operation is safe and has low postoperative complications. It can effectively promote the quality of life and the recovery of defecation function. It is worth popularizing in clinical practice.
