【摘要】：研究目的：通過觀察治療前后小兒泄瀉繼發(fā)乳糖不耐受主要療效指標的變化,客觀評價運脾止瀉顆粒治療小兒泄瀉繼發(fā)乳糖不耐受的臨床療效。 研究方法：將符合小兒泄瀉繼發(fā)乳糖不耐受的60例患兒隨機分為兩組,治療組和對照組各30例,治療組予中藥顆粒劑敷臍,以蒼術(shù)、蘇梗、葛根、焦山楂為主方,根據(jù)臨床辨證脾虛證加入木香、草果,濕熱證加入黃芩、地錦草,傷食證加入神曲、雞內(nèi)金,風(fēng)寒證加入羌活、佩蘭、皂角刺。對照組予礞脫石散聯(lián)合思連康治療。兩組均以5天為1個療程,治療并觀察1個療程。觀察兩組在治療前后大便次數(shù)、大便質(zhì)地、大便量、腹脹、嘔吐以及食欲等癥狀積分的改善情況。 研究結(jié)果：共收集60例符合要求的觀察病例,治療組和對照組各30例。治療前兩組在性別、年齡、病程、癥狀積分、病情分度等方面的比較,其差異無統(tǒng)計學(xué)意義(P0.05),具有可比性。治療組30例,痊愈7例(23.30%),顯效11例(36.7%),有效10例(33.30%),無效2例(6.70%)；對照組30例,痊愈3例(10.00%),顯效3例(10.00%),有效15例(50.00%),無效9例(30.00%)。治療組總有效率93.30%,痊愈顯效率60.00%；對照組總有效率70.00%,痊愈顯效率20.00%,治療組在總有效率、痊愈顯效率上顯著優(yōu)于對照組,有統(tǒng)計學(xué)意義(P0.01)。治療后兩組在總積分、主癥積分及次癥積分方面比較,差異有顯著的統(tǒng)計學(xué)意義(P0.01),治療組優(yōu)于對照組；其中兩組在大便次數(shù)、大便量、腹脹、嘔吐等方面比較,差異具有統(tǒng)計學(xué)意義(P0.05),治療組優(yōu)于對照組；然而在大便質(zhì)地、食欲方面的比較治療組與對照組沒有明顯差異(P0.05)。兩組治療期間均未出現(xiàn)不良反應(yīng)。 結(jié)論：運脾止瀉顆粒治療小兒泄瀉繼發(fā)乳糖不耐受的療效優(yōu)于對照組,而且無明顯副作用,是有效、安全的方法,值得臨床推廣應(yīng)用。
[Abstract]:Objective: to evaluate the clinical efficacy of Yunpi Zhixie granule in treating diarrhea secondary lactose intolerance in children by observing the changes of main therapeutic indexes before and after treatment. Methods: sixty children with lactose intolerance secondary to diarrhea were randomly divided into two groups: treatment group (n = 30) and control group (n = 30). According to clinical differentiation of spleen deficiency syndrome add wood incense, grass fruit, damp-heat syndrome to add Scutellaria baicalensis, Scutellaria chinensis, food injury syndrome to add Shenqu, chicken gold, wind and cold syndrome to add Notopterygium, Perrin, saponin thorn. The control group was treated with Mengtou Shi San and Silenkang. Both groups were treated with 5 days as a course of treatment and observed a course of treatment. The improvement of defecation times, stool texture, stool quantity, abdominal distension, vomiting and appetite were observed before and after treatment. Results: a total of 60 cases were collected, 30 cases in the treatment group and 30 cases in the control group. There was no significant difference between the two groups in sex, age, course of disease, symptom score, disease grade and so on before treatment (P0.05). In the treatment group, 7 cases (23.30%) were cured, 11 cases (36.7%) were effective, 10 cases (33.30%) were effective, and 2 cases (6.70%) were ineffective, while in the control group, 3 cases (10.00%) were cured, 3 cases (10.00%) were effective, 15 cases (50.00%) were effective, 9 cases (30.00%) were ineffective. The total effective rate of the treatment group was 93.30%, the markedly effective rate of the cure group was 60.000.The total effective rate of the control group was 70.00and the effective rate of the cure group was 20.000.The total effective rate of the treatment group was significantly better than that of the control group (P0.01). After treatment, there were significant differences in total score, main symptom score and secondary symptom score between the two groups (P0.01), the treatment group was better than the control group, and the two groups were compared in defecation times, stool volume, abdominal distension, vomiting and so on. The difference was statistically significant (P0.05), the treatment group was better than the control group; however, there was no significant difference between the treatment group and the control group in stool texture and appetite (P0.05). There were no adverse reactions during treatment in both groups. Conclusion: the efficacy of Yunpi Zhixie granule in treating lactose intolerance secondary to diarrhea in children is better than that in control group, and it is an effective and safe method with no obvious side effects. It is worth popularizing in clinic.
