【摘要】：目的:探討改良Bianchi(陰囊中縫切口)睪丸下降固定術(shù)與傳統(tǒng)(腹股溝聯(lián)合陰囊切口)睪丸下降固定術(shù)的優(yōu)缺點。方法:2013年2月至2014年2月收治的所有82例中低位隱睪患兒中,隨機對其中46例(53側(cè)睪丸)行改良Bianchi睪丸下降固定術(shù)作為觀察組;傳統(tǒng)方法即經(jīng)腹股溝聯(lián)合陰囊切口治療36例(40側(cè))作為對照組。比較2組手術(shù)時間、術(shù)后并發(fā)癥等方面的差異。結(jié)果:觀察組與對照組患兒的手術(shù)時間分別為(25±6)min和(35±4)min,兩組比較差異有統(tǒng)計學意義(P0.01);經(jīng)過1~2年的隨訪比較,均未出現(xiàn)睪丸萎縮情況;觀察組患兒出現(xiàn)3例睪丸上縮情況,對照組出現(xiàn)2例術(shù)后睪丸上縮,發(fā)生率的差異沒有統(tǒng)計學意義;術(shù)后均未出現(xiàn)腹股溝斜疝、鞘膜積液的并發(fā)癥,但對照組患兒有1例出現(xiàn)腹股溝處切口血腫且所有患兒切口均較明顯,而觀察組術(shù)后手術(shù)切口幾乎觀察不到。結(jié)論:行改良Bianchi睪丸下降固定術(shù)治療中低位隱睪效果滿意,較傳統(tǒng)手術(shù)在手術(shù)時間和切口美觀等方面具有一定的優(yōu)勢,值得臨床推廣。
[Abstract]:Objective: To investigate the advantages and disadvantages of modified Bianchi (scrotal suture incision) and traditional (inguinal combined scrotal incision) orchid descent fixation. Methods: From February 2013 to February 2014, all 82 children with middle and low cryptorchidism were randomly treated with modified Bianchi orchid descent fixation in 46 of them (53 testes). The operation time and postoperative complications of the two groups were compared. Results: The operation time of the observation group and the control group were (25 There was no testicular atrophy in the observation group; there were 3 cases of testicular atrophy in the observation group and 2 cases of testicular atrophy in the control group, the difference was not statistically significant; there were no complications of indirect inguinal hernia and hydrocele in the sheath after operation, but there was 1 case of incision hematoma in the control group and all incisions were obvious. Conclusion: Modified Bianchi testicular descent fixation has a satisfactory effect in the treatment of middle and low cryptorchidism, which is superior to traditional surgery in terms of operation time and aesthetic incision, and is worthy of clinical promotion.
