【摘要】：目的:探討雙縫合錨釘技術雙束解剖重建內(nèi)側髕股韌帶治療青少年髕股關節(jié)不穩(wěn)定的臨床療效。方法:2013年1月至2014年12月對25例青少年髕股關節(jié)不穩(wěn)定患者行雙縫合錨釘技術雙束解剖重建內(nèi)側髕股韌帶。髕骨側采用2枚帶線錨釘固定移植物,同時行髕骨側止點加強縫合,關節(jié)鏡下調(diào)節(jié)張力后采用界面擠壓螺釘固定移植物于股骨隧道。術后隨訪,內(nèi)容包括患者滿意度、髕骨外推恐懼試驗、再脫位率、CT評價骨道位置及髕骨傾斜角,采用Lysholm,Tegner和Kujala評分評價患側膝關節(jié)功能。結果:25例患者均得到隨訪,隨訪20~40(平均24)個月。髕骨恐懼試驗均為陰性,無1例感染、再脫位及髕骨骨折。CT顯示重建骨隧道位置良好,髕骨傾斜角術前為21.6°±2.3°,術后為10.5°±1.6°;Lysholm評分術前為51.7±5.3,術后為93.8±6.5;Tegner評分術前為4.1±1.1,術后為5.5±0.6;Kujala主觀評分術前為53.5±6.4,術后為94.6±4.3,差異均有統(tǒng)計學意義(P0.05)。結論:雙縫合錨釘技術雙束解剖重建內(nèi)側髕股韌帶治療青少年髕股關節(jié)不穩(wěn)定是一種安全、有效和微創(chuàng)的手術方法。
[Abstract]:Objective: to investigate the clinical effect of double suture anchor nail technique for reconstruction of medial patellofemoral ligament in juvenile patellofemoral joint instability. Methods: from January 2013 to December 2014, 25 young patients with unstable patellofemoral joint underwent double suture Anchor technique to reconstruct medial patellofemoral ligament. The patellar side was fixed with two wire anchor nails, and the patellar side was sutured with the side of the patella. After adjusting the tension under arthroscopy, the graft was fixed with interfacial compression screw in the femoral tunnel. Postoperative follow-up included patient satisfaction, patellar extrapolation fear test, redislocation rate and CT evaluation of the location of the osseous canal and patellar obliquity angle. Lysholm Tegner and Kujala scores were used to evaluate the knee joint function of the affected side. Results all 25 patients were followed up for 20 to 40 months (mean 24 months). The fear test of patella was negative, no infection was found in all cases, redislocation and patellar fracture. Ct showed that the reconstructed bone tunnel was in good position. The slope angle of patella was 21.6 擄鹵2.3 擄before operation, 10.5 擄鹵1.6 擄after operation, 51.7 鹵5.3 before operation, 93.8 鹵6.5 Tegner before operation, 5.5 鹵0.6 Kujala subjective score before operation and 94.6 鹵4.3 after operation (P0.05). Conclusion: double suture anchor nail technique is a safe, effective and minimally invasive surgical method for the treatment of juvenile patellofemoral joint instability with double bundle anatomical reconstruction of medial patellofemoral ligament.
