【摘要】：目的:對比分析早產(chǎn)與足月新生兒壞死性小腸結(jié)腸炎(neonatal necrotizing enterocolitis,NEC)的臨床特點(diǎn)。方法:回顧性分析338例NEC臨床資料,比較患兒圍產(chǎn)期相關(guān)信息、NEC診斷后并發(fā)癥、治療情況及預(yù)后危險(xiǎn)因素。結(jié)果:早產(chǎn)組94例,足月組244例,早產(chǎn)組胎膜早破發(fā)生率高于足月組;早產(chǎn)組NEC診斷后并發(fā)敗血癥、呼吸衰竭高于足月組;早產(chǎn)組NEC診斷后多巴胺、IVIG使用率及輸注紅細(xì)胞高于足月組(均P0.05)。logistic回歸分析發(fā)現(xiàn),影響早產(chǎn)組預(yù)后不良的危險(xiǎn)因素為Ⅲ期NEC(OR=6.725,95%CI=2.153~21.01,P=0.001)及并發(fā)敗血癥(OR=3.78,95%CI=1.204~11.873,P=0.023);足月組為Ⅲ期NEC(OR=7.762,95%CI=3.392~15.23,P=0.000)。結(jié)論:NEC在早產(chǎn)、足月兒人群發(fā)病的圍產(chǎn)期因素、并發(fā)癥、預(yù)后危險(xiǎn)因素方面存在差異,早產(chǎn)兒并發(fā)癥更多。Ⅲ期NEC是導(dǎo)致患兒預(yù)后不良的共同危險(xiǎn)因素,并發(fā)敗血癥是導(dǎo)致早產(chǎn)患兒預(yù)后不良的額外危險(xiǎn)因素。
[Abstract]:Objective: to compare the clinical features of (neonatal necrotizing enterocolitis (NEC) between preterm and term neonates with necrotizing enterocolitis. Methods: the clinical data of 338 cases of NEC were analyzed retrospectively. The related information of perinatal period and complications, treatment and prognostic risk factors of NEC were compared. Results: the incidence of premature rupture of membranes in premature delivery group was higher than that in term group, 94 cases in premature delivery group and 244 cases in term group, the incidence of premature rupture of membranes was higher in preterm group than in term group, and the incidence of septicemia and respiratory failure in preterm group was higher than that in term group.
