【摘要】：新生兒膿毒血癥(neonatal sepsis,NS)是新生兒期的危重癥,發(fā)病率為1~10/1000活產(chǎn)兒,病死率1%~70%不等,存在區(qū)域差異及不同新生兒人群特點;NS病原菌譜呈現(xiàn)動態(tài)變遷,目前凝固酶陰性葡萄球菌為新生兒重癥監(jiān)護室分離出的最常見的病原菌,非B族溶血性鏈球菌的病原體引發(fā)的早發(fā)型NS的發(fā)生率有所上升;NS臨床特點是非特異性癥狀及個體化臨床表現(xiàn),常易被漏診和誤診;近年來NS的檢查方法和治療措施存在的新研究進展,可以為NS的早期診斷和精確治療提供幫助;早產(chǎn)兒、低出生體重兒及其母親感染性疾病的新生兒是防治NS需關(guān)注的高危人群;NS相關(guān)的醫(yī)院感染控制方法及管理策略是未來研究重點之一;多學科研究結(jié)果需在統(tǒng)一的診斷標準下進行整合,持續(xù)推進臨床NS診斷治療技術(shù)的進步。
[Abstract]:Neonatal sepsis NS (neonatal sepsis NS) is a critical disease in neonatal stage. The incidence rate of neonatal sepsis is 110 / 1000 live infants, the mortality rate varies from 1 to 70%. There are regional differences and the characteristics of different newborns are characterized by dynamic changes of NS pathogenic bacteria spectrum. Currently coagulase-negative staphylococcus is the most common pathogen isolated from neonatal intensive care unit. The incidence of early-onset NS caused by pathogens of non-B group hemolytic streptococcus increased. The clinical characteristics of NS were nonspecific symptoms and individualized clinical manifestations, which were often missed and misdiagnosed. In recent years, the new research progress in the examination methods and treatment measures of NS can provide help for the early diagnosis and accurate treatment of NS. Low birth weight infants and their newborns with infectious diseases of their mothers are the nosocomial infection control methods and management strategies related to NS which should be paid attention to in the prevention and treatment of NS. Multidisciplinary research results need to be integrated under the unified diagnostic criteria to continuously promote the progress of clinical NS diagnosis and treatment technology.
