兒童肺炎危險因素文獻研究及臨床相關因素的Logistic回歸分析

【摘要】:目的:本文通過近20年兒童肺炎文獻中危險因素的分析研究,應用兒童肺炎危險因素調(diào)查登記表,進行臨床資料收集、整理及分析,探尋兒童肺炎的危險及保護因素,為下一步兒童肺炎防治策略提供一定科學依據(jù)。方法:利用中國生物醫(yī)學文獻網(wǎng)絡版數(shù)據(jù)庫(CBM)以關鍵詞“肺炎”、“兒”、“因素”、“危險”、“高!痹谖墨I中檢索獲取題錄,并利用國內(nèi)三大權威數(shù)據(jù)庫中國學術期刊全文數(shù)據(jù)庫(CNKI)、維普《中文科技期刊數(shù)據(jù)庫》(CQVIP)、萬方中國學術期刊數(shù)據(jù)庫(CSPD),收集1996-2016年國內(nèi)醫(yī)學期刊上公開發(fā)表的有關兒童肺炎危險因素的文獻,運用Note Express軟件將重復文獻剔除,獲得全文2038篇,將符合文獻納入標準的107篇文獻進行危險因素出現(xiàn)情況統(tǒng)計分析,將相同的危險因素歸類,按出現(xiàn)頻次排序。然后基于文獻研究的危險因素制定兒童肺炎危險因素調(diào)查登記表,自2015年12月-2016年11月,每季度調(diào)查100例前來遼寧中醫(yī)藥大學附屬醫(yī)院及大連市兒童醫(yī)院就診的兒童,去除信息不全及不符合問卷調(diào)查納入標準者8例,將符合納入標準的392例問卷調(diào)查信息建立數(shù)據(jù)庫,采用Logistic回歸分析方法,將單因素分析中有統(tǒng)計學意義(P0.05)的危險因素納入多因素回歸分析中,根據(jù)多因素Logistic回歸分析結果中B(回歸系數(shù))及OR(比數(shù)比)確定兒童肺炎的危險因素及保護因素。結果:1.文獻研究中入選107篇文獻,43篇文獻來源于核心期刊,62篇屬于統(tǒng)計源期刊,2篇碩士論文,出現(xiàn)頻次前5位的危險因素為年齡、環(huán)境污染(包括室內(nèi)與室外)、先心病、季節(jié)氣候、營養(yǎng)不良。2.多因素Logistic回歸分析顯示被動吸煙、反復呼吸道感染、大氣污染、先天性疾病B0,OR1,為兒童肺炎的危險因素;合理飲食、室內(nèi)裝修5年以上、嬰兒期母乳喂養(yǎng)B0,OR1,是兒童肺炎的保護因素。結論:1.文獻研究顯示年齡(嬰幼兒多見)、環(huán)境污染(包括室內(nèi)與室外)、先心病、季節(jié)氣候、營養(yǎng)不良是兒童常見的危險因素,其他出現(xiàn)頻次較低的因素為早產(chǎn)、個人過敏史及免疫功能異常等50項。2.問卷調(diào)查研究顯示被動吸煙、反復呼吸道感染、大氣污染、先天性疾病是兒童肺炎的危險因素;合理飲食、室內(nèi)裝修5年以上、嬰兒期母乳喂養(yǎng)是兒童肺炎的保護因素。
[Abstract]:Objective: through the analysis and study of the risk factors in the literature of children's pneumonia in the last 20 years, this paper applies the survey registration form of the risk factors of children's pneumonia, and carries out the clinical data collection, collation and analysis to explore the risk and protective factors of children's pneumonia, and to provide a scientific basis for the next strategy of prevention and control of pneumonia in children. The document network version database (CBM) uses key words "pneumonia", "son", "factor", "risk", "high-risk" to retrieve the title in the literature, and uses the three major authoritative database of China's Chinese academic journal full text database (CNKI), VP < Chinese Sci-tech Journal Database > (CQVIP), Wan Fang Chinese academic journal database (CSPD), and collect 1 The literature on the risk factors of children pneumonia published in the domestic medical journal in 996-2016 years, using Note Express software to remove the repeated documents and obtain the full text of 2038 articles, which will conform to the standard 107 documents to analyze the occurrence of risk factors, classify the same risk factors and sort out the occurrence frequency. Based on the risk factors of literature study, the registration form of the risk factors for children's pneumonia was made. From November -2016 December 2015, 100 children who came to the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine and Dalian children's hospital were investigated each quarter, and 8 cases were removed from the information of incomplete information and those who did not conform to the questionnaire, which would be in line with the standard of 392. The database was set up by the questionnaire survey information. The Logistic regression analysis was used to integrate the risk factors of statistical significance (P0.05) in the single factor analysis into multiple factor regression analysis. The risk factors and protective factors of children pneumonia were determined according to the B (regression coefficient) and OR (ratio Ratio) in the results of multi factor Logistic regression analysis. The risk factors for the first 5 were age, environmental pollution (including indoor and outdoor), congenital heart disease, seasonal climate, and malnutrition.2. multiple factors Logistic regression analysis showed passive smoking, recurrent respiratory infection, Gas pollution, congenital disease B0, OR1, is a risk factor for children pneumonia; reasonable diet, indoor decoration for more than 5 years, infant breastfeeding B0, OR1, is a protective factor for children pneumonia. Conclusion: 1. literature studies show that age (many children), environmental pollution (including indoor and outdoor), congenital heart disease, seasonal climate, malnutrition are common children The risk factors and other low frequency factors were preterm labor, individual allergy history and immune function abnormality in 50.2. questionnaires, which showed that passive smoking, recurrent respiratory infection, air pollution, and congenital disease were risk factors for children pneumonia; reasonable diet, indoor decoration for more than 5 years and infant breastfeeding were the protection of pneumonia in children.
