【摘要】：目的：觀察大桑菊飲聯(lián)合阿奇霉素治療小兒肺炎支原體支氣管炎風熱證的臨床療效并進行客觀檢驗,在此基礎(chǔ)上,探討大桑菊飲治療小兒肺炎支原體支氣管炎風熱證的作用及中西藥聯(lián)合功效互補的意義,為治療小兒肺炎支原體支氣管炎尋求比較好的治療方法。 方法： 本研究設(shè)立治療組和對照組兩組,分組采用前瞻性簡單隨機試驗法,將符合納入標準的60例患者隨機分入兩組,其中治療組30例、對照組30例。治療組采用大桑菊飲聯(lián)合阿奇霉素方案進行治療,對照組采用單用阿奇霉素方案治療,療程均為21天。治療前后將對臨床觀察癥狀體征評分,并利用統(tǒng)計工具對治療組與對照組治療前后的癥狀體征及MP-IgM指標進行對比分析。 成果： 1、癥狀與體征改善方面,Rdit檢驗與卡方檢驗表明,在咳嗽、咯痰癥狀方面相關(guān)的P值0.05,治療組與對照組比較有顯著性差異,表明治療組在改善咳嗽、咯痰癥狀方面優(yōu)于對照組,在退熱、肺部體征改善方面相關(guān)P值0.05,,無顯著性差異,表明治療組與對照組療效相當。 2、治療組和對照組主要癥狀消失時間比較,經(jīng)統(tǒng)計學(xué)檢驗,兩組在咳嗽與咯痰消失時間方面存在顯著性差異(P0.05),表明大桑菊飲加阿奇霉素分散片在咳嗽、咯痰癥狀消除時間方面優(yōu)于單獨使用阿奇霉素分散片。 3.MP-IgM指標方面,Fish的精確檢驗的P值明顯大于0.05,證明治療組與對照組在改善MP-IgM值方面無顯著性差異。 4、治療組治療后痊愈、顯效、有效、無效的病例數(shù)為4例、20例、5例、1例,總有效率為96.6%,對照組治療后痊愈、顯效、有效、無效的病例數(shù)為2例、17例、8例、3例,總有效率為90%,經(jīng)統(tǒng)計學(xué)檢驗P0.05,兩組總有效率不存在顯著性差異。 結(jié)論： 大桑菊飲聯(lián)合阿奇霉素治療小兒肺炎支原體支氣管炎風熱證療效優(yōu)于單純阿奇霉素治療。大桑菊飲治療小兒肺炎支原體支氣管炎風熱證療效顯著。
[Abstract]:Objective: to observe the clinical effect of Dasangju decoction combined with azithromycin in treating mycoplasma pneumoniae mycoplasma bronchitis wind and heat syndrome and to carry out objective test. To explore the effect of Dasangju decoction in treating mycoplasma pneumoniae mycoplasma bronchitis wind and heat syndrome and the significance of complementary effect of traditional Chinese and western medicine in the treatment of mycoplasma pneumoniae mycoplasma bronchitis in children. Methods: in this study, the treatment group and the control group were divided into two groups, 30 cases in the treatment group and 30 cases in the control group, and 60 patients who met the inclusion criteria were randomly divided into two groups. The treatment group was treated with Dasangju decoction combined with azithromycin, while the control group was treated with azithromycin alone for 21 days. The clinical symptoms and signs were scored before and after treatment, and the symptoms and signs and MP-IgM indexes of the treatment group and the control group were compared and analyzed by statistical tools before and after treatment. Results: 1. The results of Rdit test and chi-square test showed that there was significant difference between the treatment group and the control group in terms of cough and sputum symptoms (P = 0.05), indicating that the treatment group was improving the cough. The symptoms of expectoration were better than that of the control group (P = 0.05). There was no significant difference between the treatment group and the control group, indicating that the treatment group had the same curative effect as the control group, and the main symptoms disappeared in the treatment group and the control group. By statistical test, there was a significant difference between the two groups in the time of cough and sputum disappearance (P0.05), indicating that Dasangju Yin and Azithromycin dispersible tablets were coughing, The time of eliminating sputum symptoms was better than that of using azithromycin dispersible tablets alone. 3. The precise test of Fish in the index of MP-IgM was significantly greater than 0.05, which proved that there was no significant difference in improving MP-IgM between the treatment group and the control group. The treatment group was cured after treatment. The effective, effective and ineffective cases were 4 cases (20 cases), 20 cases (5 cases) and 1 case (1 case). The total effective rate was 96. 6%. In the control group, the patients were cured, effective and effective. The number of ineffective cases was 2 cases (17 cases), 17 cases (8 cases) and 3 cases (3 cases). The total effective rate was 90. There was no significant difference in the total effective rate between the two groups by statistical test (P0.05). Conclusion: Dasangju decoction combined with azithromycin is superior to azithromycin in the treatment of mycoplasma pneumoniae mycoplasma bronchitis in children. Dasangju decoction is effective in treating mycoplasma pneumoniae mycoplasma in children with wind-heat syndrome.
