【摘要】：目的：評估術(shù)前化療對兒童腎母細(xì)胞瘤腫瘤縮小率、腫瘤全切率及術(shù)后無瘤生存率影響，為進(jìn)一步提高臨床治療效果及研究水平提供參考。 方法：根據(jù)制定好的檢索策略，計算機(jī)檢索PubMed，Cochrane（2011年第3期），OVID（包括EMbase和Biosis previews數(shù)據(jù)庫），VIP，CNKI，Wang Fang Data，由2名評價員按納入排除標(biāo)準(zhǔn)獨(dú)立選擇文獻(xiàn)、提取數(shù)據(jù)并交叉核對，依據(jù)Cochrane handbook5.0.0推薦的4條隨機(jī)對照試驗(yàn)質(zhì)量評價標(biāo)準(zhǔn)評價納入研究的方法學(xué)質(zhì)量，并采用Stata11軟件進(jìn)行統(tǒng)計分析。 結(jié)果：最終納入30個研究,研究存在中度或高度偏倚的可能性。Meta分析結(jié)果顯示，術(shù)前化療加手術(shù)治療與單純手術(shù)比較，能提高腎母細(xì)胞瘤腫瘤縮小率、腫瘤全切率和術(shù)后無瘤生存率，兩組差異有統(tǒng)計學(xué)意義[分別為OR=37.28，95%CI(20.18，68.81)，P=0.0000；OR=5.923，95%CI(3.14，10.95)，，P=0.000；OR=2.03，95%CI(1.58，2.60)，P=0.000]。 結(jié)論：術(shù)前化療能提高兒童腎母細(xì)胞瘤腫瘤縮小率、腫瘤全切率和可能提高術(shù)后無瘤生存率。
[Abstract]:Objective: to evaluate the effect of preoperative chemotherapy on tumor reduction rate, total tumor resection rate and tumor free survival rate in children with nephroblastoma. Methods: according to the established search strategy, the online search was performed on PubMedan Cochrane (issue 3, 2011), which included Embase and Biosis previews databases. Two evaluators independently selected the literature according to the exclusion criteria, extracted the data and cross-checked the data. The methodological quality of four randomized controlled trials recommended by Cochrane handbook5.0.0 was evaluated and statistically analyzed by Stata11 software. Results: the possibility of moderate or high bias was found in 30 studies. Meta-analysis showed that preoperative chemotherapy plus surgical treatment could increase the tumor size of nephroblastoma compared with surgery alone. The total tumor resection rate and the postoperative tumor-free survival rate were significantly different between the two groups [CI (20.18 鹵68.81) P 0.0000] 5.92395 CI (3.14 鹵10.95) P 0.000 OR 2.03C _ (95) CI (1.582.60) P0. 000). Conclusion: preoperative chemotherapy can improve tumor reduction rate, total tumor resection rate and tumor free survival rate of children with nephroblastoma.
