【摘要】：目的:觀察不同方案治療兒童眼肌型重癥肌無力(myasthenia gravis,MG)的效果、復(fù)發(fā)影響因素及向全身型MG轉(zhuǎn)化的轉(zhuǎn)化率。方法:對2010年6月至2014年3月于中南大學湘雅醫(yī)院小兒神經(jīng)�？凭驮\的104例兒童眼肌型MG患者進行回顧性臨床研究。按治療方案分為甲基強的松龍組(n=44)、強的松組(n=48)和單用溴吡斯的明組(n=12)。采用美國MG基金會干預(yù)后狀態(tài)評價系統(tǒng)評價治療有效率、向全身型MG轉(zhuǎn)化的轉(zhuǎn)化率。結(jié)果:甲基強的松龍組較強的松組短期內(nèi)的治療有效率提高,這兩組患者的有效率均高于單用溴吡斯的明組(均P0.05)。聯(lián)合使用甲基強的松龍、強的松和溴吡斯的明治療兒童眼肌型MG的遠期療效較好。早期糖皮質(zhì)激素治療可有效減少復(fù)發(fā)。結(jié)論:大劑量甲基強的松龍沖擊治療對于眼肌型MG患兒早期癥狀緩解起積極作用,而不同方案的糖皮質(zhì)激素治療遠期療效相似,起病年齡大、感染、甲狀腺功能異常的兒童眼肌型MG患者復(fù)發(fā)率較高。
[Abstract]:Objective: to observe the effect of different regimens on myasthenia gravis MGs in children with ocular myasthenia gravis, the factors affecting recurrence and the transformation rate to systemic MG. Methods: from June 2010 to March 2014, 104 children with ocular myometric MG who were admitted to Xiangya Hospital of Central South University for pediatric neurology were studied retrospectively. The patients were divided into two groups: methylprednisolone group (n = 44), prednisone group (n = 48) and brompimide group (n = 12). The effective rate of treatment and the transformation rate to systemic MG were evaluated by the state evaluation system after intervention by the MG Foundation of USA. Results: the effective rate of methylprednisolone group was higher than that of prednisone group in a short period of time. Combined use of methylprednisolone, prednisone and brompiridamine in the treatment of myometric MG in children had good long-term results. Early glucocorticoid therapy can effectively reduce recurrence. Conclusion: high dose methylprednisolone pulse therapy plays a positive role in relieving the early symptoms of myometric MG children, while the long-term effect of glucocorticoid therapy with different regimens is similar, the onset age of the disease is older and the infection occurs. The recurrence rate of myometric MG in children with abnormal thyroid function was higher.
