【摘要】：研究目的:探討肥胖少年頸圍(NC)、腰圍(WC)與心肺耐力(CRF)及心血管疾病(CVD)危險(xiǎn)因素的關(guān)系,探討CRF對(duì)心血管健康的影響,為心血管疾病的早期預(yù)防提供科學(xué)依據(jù)。研究方法:招募8~17歲肥胖少年57名(男25人,女32人),平均年齡13.5歲。使用皮尺對(duì)所有受試者進(jìn)行NC、WC測(cè)量。功率自行車遞增負(fù)荷試驗(yàn)測(cè)量心肺耐力(CRF),CRF水平通過氣體代謝分析儀所測(cè)定的峰值攝氧量表示。測(cè)量安靜收縮壓(SBP)、舒張壓(DBP),檢測(cè)空腹血糖(FPG)、總膽固醇(TC)、甘油三酯(TG)、高密度脂蛋白膽固醇(HDLC)、低密度脂蛋白膽固醇(LDL-C)。結(jié)果:調(diào)整年齡、性別后,NC、WC、腰圍身高比(WHt R)均與SBP、DBP、TG正相關(guān),均與CRF負(fù)相關(guān);NC、WC均與HDL負(fù)相關(guān)。調(diào)整年齡、性別、CRF后,NC是高SBP、高TG、低HDL-C的獨(dú)立危險(xiǎn)因素,OR值分別為1.278(95%CI:1.027,1.590)、1.342(95%CI:1.017,1.772)、1.287(95%CI:1.042,1.589),WC是高SBP的獨(dú)立危險(xiǎn)因素,OR值為1.095(95%CI:1.013,1.185),但是WC對(duì)高TG的預(yù)測(cè)效果受到CRF的影響。調(diào)整年齡、性別后,CRF與TG負(fù)相關(guān),對(duì)TG變異的解釋增加12.1%(P0.05)。與沒有CVD危險(xiǎn)因素者相比,具有1個(gè)以上危險(xiǎn)因素者,NC、WC、WHt R均顯著較高(P0.05),CRF顯著較低(P0.05)。結(jié)論:肥胖少年的頸圍、腰圍與血壓、血脂關(guān)系密切,但關(guān)聯(lián)程度受到心肺耐力的調(diào)節(jié),心肺耐力是血脂水平的保護(hù)因素。此外,頸圍、腰圍對(duì)心肺耐力也具有預(yù)測(cè)作用,頸圍、腰圍越小,心肺耐力越好。建議今后對(duì)肥胖少年的相關(guān)研究需要重視頸圍、腰圍的測(cè)量,重視心肺耐力對(duì)保護(hù)心血管健康的作用。
[Abstract]:Objective: to investigate the relationship between the risk factors of the obesity juvenile neck circumference (NCN), waist circumference (WCC) and cardiopulmonary endurance (CRF) and cardiovascular disease (CVD), to explore the effect of CRF on cardiovascular health, and to provide scientific basis for early prevention of cardiovascular disease. Methods: 57 obese adolescents (25 males and 32 females) were recruited, with an average age of 13.5 years. All subjects were measured by NCU WC with a caliper. The level of CRF and CRF measured by increasing load test was expressed by the peak oxygen uptake measured by gas metabolism analyzer. Systolic blood pressure (SBP), diastolic blood pressure (DBP), fasting blood glucose (FPG), total cholesterol (TC), triglyceride (TGN), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C) were measured. Results: after adjustment of age, sex, NCU WC, waist circumference / height ratio (WHT R) were all positively correlated with SBP, DBP, TG, and negatively correlated with CRF, NCU WC and HDL. After adjusting for age, the independent risk factors of high SBP, high TG, and low HDL-C were 1.278 鹵95CI: 1.027272.9590 CI: 1.017 1. 772 1. 772CI: 1. 28795 CIW 1.0421.589WC was the independent risk factor of high SBP or 1.095 95% CI 1.0131.185, but the prediction effect of WC on high TG was affected by CRF. CRF was negatively correlated with TG, and the explanation of TG variation increased by 12.1g / P0.05a. Compared with those without CVD risk factors, the WHT R of NCU WCU was significantly higher in those with more than one risk factor, and the CRF was significantly lower in those with more than one risk factor (P 0.05). Conclusion: the neck circumference and waist circumference of obese adolescents are closely related to blood pressure and blood lipids, but the degree of correlation is regulated by cardiopulmonary endurance, which is the protective factor of blood lipid level. In addition, neck circumference and waist circumference also predict cardiopulmonary endurance. The smaller the neck circumference and waist circumference, the better cardiopulmonary endurance. It is suggested that future research on obese adolescents should pay attention to the measurement of neck circumference and waist circumference and the role of cardiopulmonary endurance in protecting cardiovascular health.
