早產(chǎn)兒與足月兒精細(xì)運動發(fā)育差異及影響因素的研究

【摘要】:目的了解早產(chǎn)兒與足月兒精細(xì)運動發(fā)育水平的差異及其影響因素。 方法采用Peabody運動發(fā)育量表第二版(Peabody DevelopmentalMotor Scales2即PDMS-2)對215例嬰兒(早產(chǎn)兒97例、足月兒118例)進(jìn)行精細(xì)運動發(fā)育水平測試,并對所得結(jié)果精細(xì)運動發(fā)育商(FMQ)進(jìn)行統(tǒng)計分析。 結(jié)果1)精細(xì)運動發(fā)育的抓握原始分和視覺運動整合原始分均隨月齡的增加而增加,不同年齡段FMQ(矯正前)、FMQ(矯正后)、FMQ(足月)不同,(F=25.135P<0.05;F=28.634P<0.05;F=12.151P<0.05);2)早產(chǎn)兒FMQ(矯正前)89.58±5.43顯著小于足月兒FMQ(足月)93.90±6.65(t=5.142P<0.05);3)精細(xì)運動的抓握標(biāo)準(zhǔn)分低于視覺運動整合標(biāo)準(zhǔn)分,(d=-1.00±1.24,t=-11.881P<0.05);4)早產(chǎn)兒的抓握和視覺運動整合的標(biāo)準(zhǔn)分均落后于足月兒,(t=4.695P<0.05;t=4.385P<0.05);5)無論早產(chǎn)兒還是足月兒,,均有抓握相當(dāng)月齡和視覺運動整合相當(dāng)月齡小于嬰兒月齡,差異有統(tǒng)計學(xué)意義(P<0.05);6)FMQ受到胎齡、出生體重、是否試管嬰兒、進(jìn)食能力訓(xùn)練、身高、體重多方面因素的影響。 結(jié)論嬰兒期精細(xì)運動的發(fā)育隨年齡的增加不斷發(fā)展,6月-1歲期間,精細(xì)運動發(fā)育的速度逐漸減慢;早產(chǎn)兒精細(xì)運動發(fā)育落后于足月兒,且抓握能力發(fā)育較視覺運動整合能力發(fā)育落后;孕期加強保健延長胎齡增加出生體重、生后有意識通過進(jìn)食能力訓(xùn)練加強動手能力的提高、保持適宜的生長發(fā)育對精細(xì)運動的發(fā)育極其重要。
[Abstract]:Objective to investigate the difference of fine motor development between preterm and term infants and its influencing factors. Methods the fine motor development level of 215 infants (97 preterm infants and 118 full-term infants) was measured by Peabody motor development scale (PDMS-2), and the results were statistically analyzed. Results 1) the grasping primordial score and visual motor integration score of fine motor development increased with the increase of age. FMQs of preterm infants (89.58 鹵5.43 before correction) were significantly lower than those of full-term infants (93.90 鹵6.65(t=5.142P < 0.05). The grasp standard scores of preterm infants were lower than those of visual motor integration standard scores (-1.00 鹵1.24t- 11.881P < 0.05t- 11.881P < 0.05). The standard score of motor integration was lower than that of term infants (4.695 P < 0.05 P < 0.05 P < 0.05). There were significant differences in grasping age and visual motor integration between the two groups (P < 0.05). FMQ was affected by the factors of gestational age, birth weight, test-tube infant, feeding ability training, height and weight. Conclusion the development of fine motor in infancy develops continuously with the increase of age, and the rate of fine motor development slows down gradually during June 1, and the fine motor development of preterm infants lags behind that of full-term infants. The development of grip ability was backward than that of visual motor integration ability, and the health care during pregnancy prolonged the gestational age to increase birth weight, and consciously increased the practical ability through feeding ability training after birth. Maintaining proper growth and development is essential for the development of fine motion.
