【摘要】：我國(guó)白血病的發(fā)病率為十萬(wàn)分之四萬(wàn)人,每年新增白血病患者達(dá)4萬(wàn)余人,其中,新增兒童白血病患者占二分之一。針對(duì)兒童白血病群體的研究中,醫(yī)學(xué)等其它領(lǐng)域?qū)純杭捌湔疹櫿叩难芯枯^多,社會(huì)工作介入的研究較少,專(zhuān)門(mén)針對(duì)白血病患兒支持網(wǎng)絡(luò)的個(gè)案管理方法介入研究則鮮有文獻(xiàn)報(bào)道。本研究在山西某醫(yī)院選取了50名白血病患兒為研究對(duì)象,通過(guò)問(wèn)卷調(diào)查方式分別從情緒、認(rèn)知、生活秩序、資源四方面對(duì)其進(jìn)行了現(xiàn)狀分析。發(fā)現(xiàn)因疾病讓大多數(shù)患兒的人際交往、性格、脾氣、情緒狀態(tài)、就學(xué),患者的家庭氛圍、家庭成員的情緒狀態(tài)都產(chǎn)生了較大影響,而對(duì)白血病知識(shí)的了解成為多數(shù)患者照顧者的主要訴求之一,絕大多數(shù)患者獲得的主要支持和照顧均來(lái)自家庭成員,其它方面支持較少,經(jīng)濟(jì)方面受到非常嚴(yán)重的影響。在此基礎(chǔ)上,本研究以其中一名白血病患兒為例,探討了通過(guò)個(gè)案管理模式建構(gòu)社會(huì)支持網(wǎng)絡(luò)的方式來(lái)幫助其解決情緒、認(rèn)知、生活秩序、資源幾方面困境,發(fā)現(xiàn)從個(gè)人網(wǎng)絡(luò)(家庭成員的支持、親戚朋友的支持和幫助)、志愿連接(志愿者的教育支持和娛樂(lè)陪伴支持)、相互援助網(wǎng)絡(luò)(病友間的知識(shí)性支持)、社會(huì)授權(quán)網(wǎng)絡(luò)(村委會(huì)、所在醫(yī)院的醫(yī)護(hù)人員、愛(ài)心基金的救助、公益平臺(tái)的支持等)四個(gè)方面策略能夠協(xié)助白血病患者有效構(gòu)建起社會(huì)支持網(wǎng)絡(luò),而該支持網(wǎng)絡(luò)的建構(gòu)和完善能夠提高白血病患兒及其照顧者應(yīng)對(duì)問(wèn)題的能力,積極面對(duì)因重大疾病帶來(lái)的情緒、認(rèn)知、生活秩序和資源等困境,緩解因突發(fā)事件帶來(lái)的壓力感。最后,本研究分別從四個(gè)網(wǎng)絡(luò)提出了家庭支持、親戚朋友支持、志愿者教育、團(tuán)隊(duì)建立、資源利用、環(huán)境建設(shè)等對(duì)策與建議。
[Abstract]:The incidence of leukemia in China is 40000/100000, and the number of new leukemia patients is more than 40, 000 every year, of which 1/2 are new in children. In the study of childhood leukemia population, there are more researches on children and their caregivers in medicine and other fields, but less research on social work intervention. There are few reports on the intervention of case management methods for leukemia support network. In this study, 50 children with leukemia were selected from a hospital in Shanxi Province, and their current situation was analyzed from four aspects: emotion, cognition, order of life and resources. It was found that the interpersonal communication, personality, temper, emotional state, school attendance, the family atmosphere of the patients and the emotional state of the family members of most of the children were greatly affected by the disease. The understanding of leukemia knowledge has become one of the main demands of most patients' caregivers. The majority of patients are mainly supported and cared for by family members, while others are less supported, and the economic impact is very serious. On this basis, this study takes one of the children with leukemia as an example to explore the way of constructing social support network through case management model to help them solve the plight of emotion, cognition, order of life and resources. Found from personal networks (support from family members, support and assistance from relatives and friends), voluntary connections (educational support and recreational support for volunteers), mutual assistance networks (knowledge support among patients, social empowerment networks (village committees), Medical staff in the hospital, aid from the charity fund, support from the public welfare platform, etc.) four strategies can help patients with leukemia effectively build up a social support network. The construction and improvement of the support network can improve the ability of children with leukemia and their caregivers to cope with the problems, actively face the difficulties caused by major diseases, such as emotion, cognition, order of life and resources, and relieve the pressure caused by emergencies. Finally, this study put forward the countermeasures and suggestions from four networks, such as family support, relatives and friends support, volunteer education, team building, resource utilization, environmental construction and so on.
