【摘要】：目的探討新生兒呼吸窘迫綜合征(respiratory distress syndrome,RDS)早產兒宮外發(fā)育遲緩(extrauterine growth retardation,EUGR)的發(fā)生情況,并分析其相關危險因素,探討預防對策,降低其發(fā)生率,改善營養(yǎng)狀況。 方法收集2011年1月1日-2012年10月31日NICU的60例適于胎齡兒的RDS早產兒的臨床資料,根據(jù)出院時體質量與矯正胎齡的關系,分為EUGR組與非EUGR組,應用SPSS17.0軟件進行數(shù)據(jù)分析,比較兩組基本情況、圍產期因素、營養(yǎng)攝入情況、治療措施及并發(fā)癥方面的差異；并對相關因素進行Logistc回歸分析,篩選出RDS早產兒宮外發(fā)育遲緩的高危因素。 結果RDS早產兒中適于胎齡兒的宮外發(fā)育遲緩發(fā)生率為56.67%(34/60)。EUGR組與非EUGR組出生體質量、最小體質量、多胎妊娠、分娩方式差異有統(tǒng)計學意義(P0.05),在胎齡、營養(yǎng)攝入情況、治療措施及并發(fā)癥方面差異無統(tǒng)計學意義(P0.05)。Logistc回歸分析：恢復出生體重時間、多胎妊娠為危險因素,產前激素應用為保護性因素。 結論RDS早產兒中適于胎齡兒的EUGR與多胎妊娠、剖宮產、出生體質量低、最小體質量有關,恢復出生體重時間及多胎妊娠為EUGR危險因素。
[Abstract]:Objective to investigate the incidence of extrauterine growth retard (EUGRG) in premature infants with distress syndrome, analyze its risk factors, and explore preventive measures to reduce its incidence and improve nutritional status. Methods the clinical data of 60 premature infants with RDS from January 1, 2011 to October 31, 2012 in NICU were collected. According to the relationship between body mass and corrected gestational age at discharge, they were divided into EUGR group and non EUGR group. The data were analyzed by SPSS17.0 software. The factors of perinatal period, nutritional intake, therapeutic measures and complications were compared between the two groups, and the risk factors of extrauterine growth retardation of premature infants with RDS were screened by Logistc regression analysis. Results the incidences of extrauterine growth retardation in RDS premature infants were as follows: birth weight, minimum body mass, multiple pregnancy and delivery mode of 56.67%(34/60).EUGR group and non-EUGR group were significantly different (P 0.05). There was no significant difference in treatment measures and complications (P 0.05). Logistc regression analysis showed that the time of recovery of birth weight, multiple pregnancy was the risk factor, and prenatal hormone was the protective factor. Conclusion EUGR in RDS premature infants is associated with multiple pregnancy, cesarean section, low birth weight and minimum body mass. The time of recovery of birth weight and multiple pregnancy are risk factors of EUGR.
