【摘要】：目的調(diào)查初發(fā)1型糖尿病患兒酮癥酸中毒(DKA)的發(fā)生情況。方法以224例初發(fā)1型糖尿病患兒為研究對象,進(jìn)行回顧性分析,分為DKA組和未合并DKA組,各112例。DKA組患兒根據(jù)年齡分為≥5歲組(65例)和5歲組(47例),并根據(jù)酸中毒情況分為輕度(26例)、中度(29例)、重度(57例)3組。分析DKA發(fā)生的影響因素以及不同年齡DKA患兒的臨床及實(shí)驗(yàn)室特點(diǎn)。結(jié)果 224例初發(fā)1型糖尿病患兒中最常見的癥狀為多飲(86.2%)、多尿(78.6%)及體重下降(57.1%)。與未合并DKA患兒比較,DKA組5歲、低收入、父母教育程度高中及以下所占的比例均較高,隨機(jī)血糖、Hb A1C水平較高,pH、HCO_3~-及C肽水平更低,差異均具有統(tǒng)計(jì)學(xué)意義(P0.05)�！�5歲組與5歲組的輕、中、重度DKA所占比例的差異無統(tǒng)計(jì)學(xué)意義(P0.05)。與5歲組相比,≥5歲組DKA患兒的癥狀持續(xù)時(shí)間較長,隨機(jī)血糖較低,HbA1C、C肽水平較高,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論 1型糖尿病患兒DKA發(fā)生率高,DKA的發(fā)生與年齡、父母文化程度及家庭收入有關(guān)。
[Abstract]:Objective to investigate the incidence of ketoacidosis in children with type 1 diabetes mellitus. Methods 224 children with primary type 1 diabetes mellitus were retrospectively analyzed and divided into two groups: DKA group and uncomplicated DKA group. 112 cases of DKA group were divided into 5 years old group (65 cases) and 5 years old group (47 cases). According to the condition of acidosis, they were divided into 3 groups. To analyze the influencing factors of DKA and the clinical and laboratory characteristics of DKA children of different ages. Results the most common symptoms of 224 children with type 1 diabetes were polydipsia (86.2) and diuresis (78.6) and weight loss (57.1%). In DKA group, the proportion of children aged 5 years old, low income, parents' education level and HbA1C level were higher than those in children without DKA, and the levels of HbA1C and C-peptide were lower in DKA group than those in non-DKA group, and the levels of HbA1C and HbA1C were lower in DKA group than those in DKA group. The difference was statistically significant (P < 0.05). There was no significant difference in the proportion of severe DKA between the group of 鈮，
