【摘要】：目的:探討孕婦IgG效價與不同血型新生兒患溶血病(hemolytic disease of newborn,HDN)的相關(guān)性。方法:選取2014年5月-2015年1月在我院進(jìn)行產(chǎn)檢的O型血孕婦400例,且夫妻雙方血型不同,在檢查過程中對孕婦的IgG效價進(jìn)行了檢測,并且追蹤孕婦產(chǎn)后新生兒溶血病的發(fā)生情況;根據(jù)新生兒是否出現(xiàn)HDN分為兩組:HDN組和非HDN組。HDN組85例,非HDN組315例,并對IgG效價與其相關(guān)性進(jìn)行系統(tǒng)的研究分析。結(jié)果:將產(chǎn)檢過程中孕婦IgG效價分為1∶64、1∶64、1∶128、1∶256和1∶512 5組,對IgG效價64 4組的新生兒HDN發(fā)病率與IgG效價64的新生兒HDN發(fā)病率分別比較顯示,ABO-HDN的患病率分別為96.9%、79.6%、63.7%和28.8%,孕婦IgG效價與新生兒ABO溶血病的發(fā)生存在著一定的相關(guān)性,即隨著IgG效價的升高,新生兒溶血病的發(fā)病率有升高的趨勢(r=0.8832)。4組中新生兒的HDN發(fā)病的風(fēng)險均高于IgG效價64新生兒的HDN組。結(jié)論:孕婦IgG效價與ABOHDN的發(fā)病率有一定的相關(guān)性,因此對于IgG效價高的孕婦在產(chǎn)前進(jìn)行效價的控制對于降低新生兒溶血病的發(fā)病率具有重要的臨床意義。
[Abstract]:Objective: to investigate the relationship between IgG titer of pregnant women and hemolytic disease of newbornus in newborns with different blood groups. Methods: 400 pregnant women with type O blood were selected from May 2014 to January 2015, and their blood groups were different. The IgG titer of pregnant women was detected during the examination. According to the occurrence of neonatal hemolytic disease in pregnant women, HDN was divided into two groups: HDN group (n = 85), non-HDN group (n = 85) and non-HDN group (n = 315). The relationship between IgG titer and its correlation was analyzed systematically. Results: the IgG titers of pregnant women were divided into 1: 64: 1: 64: 1: 1: 128 1: 256 and 1: 512 5 groups. The incidence of HDN in neonates with IgG titer 64 and HDN in newborns with IgG titer 64 showed that the prevalence rate of ABO-HDN was 96.90.79. 6% and 28.887%, respectively. There was a certain correlation between IgG titer of pregnant women and the occurrence of ABO hemolytic disease in neonates. That is, with the increase of IgG titer, the incidence of hemolytic disease in neonates has a tendency to increase. The risk of neonatal HDN in group R 0.8832 4 is higher than that in HDN group with IgG titer of 64 neonates. Conclusion: the IgG titer of pregnant women is related to the incidence of ABOHDN, so it is of great clinical significance to control the titer of pregnant women with high IgG titer in prenatal to reduce the incidence of neonatal hemolysis.
