重慶市九龍坡區(qū)學(xué)齡兒童貧血與體格生長(zhǎng)的關(guān)系調(diào)查

【摘要】:目的:探討學(xué)齡期兒童貧血與體格生長(zhǎng)的關(guān)系。方法:在重慶市九龍坡區(qū)采用多階段分層整群隨機(jī)抽樣的方法,對(duì)5所全日制公立小學(xué)的6 260名學(xué)齡兒童進(jìn)行體格測(cè)量及血生化指標(biāo)檢查。統(tǒng)計(jì)分析貧血組及非貧血組間常用體格生長(zhǎng)指標(biāo)(體質(zhì)量、身高)和與貧血相關(guān)的血常規(guī)指標(biāo),如紅細(xì)胞平均容積(mean corpuscular volume,MCV)、紅細(xì)胞平均血紅蛋白(mean corpuscular hemoglobin,MCH)、紅細(xì)胞平均血紅蛋白濃度(mean corpuscular hemoglobin concentration,MCHC)和紅細(xì)胞分布寬度(red blood cell distribution width,RDW)的差異及相關(guān)性。結(jié)果:4 730例有效樣本中,共檢出1 140例貧血兒童,貧血的總檢出率為24.1%,女童貧血的檢出率顯著高于男童(男童檢出率為22.6%,女童檢出率為25.7%,χ2=6.100,P=0.010)。各年齡組(6歲~、7歲~、8歲~、9歲~、10歲~、11歲~、12歲~)貧血檢出率分別為29.1%、29.1%、27.6%、23.7%、20.9%、17.4%、10.5%。不同年齡組間貧血檢出率存在顯著差異(χ2=75.800,P=0.000)。貧血組與非貧血組間的體格生長(zhǎng)指標(biāo)、血常規(guī)指標(biāo)均存在顯著性差異(P0.05)。貧血對(duì)兒童的體質(zhì)量及身高均具有顯著影響(分別為F=124.400,P=0.000;F=93.650,P=0.000)。血紅蛋白(Hb)濃度與體質(zhì)量、身高、體質(zhì)指數(shù)(body mass index,BMI)、MCV、MCH、MCHC均具有顯著相關(guān)性(相關(guān)系數(shù)分別為0.233、0.221、0.174、0.277、0.427、0.319,P值均為0.000)。結(jié)論:重慶市九龍坡區(qū)學(xué)齡兒童貧血的發(fā)生率較高;貧血的發(fā)生與年齡、性別、體質(zhì)量和身高均存在相關(guān)關(guān)系;貧血對(duì)學(xué)齡期兒童的體質(zhì)量及身高均具有顯著影響。提示重慶市九龍坡區(qū)學(xué)齡兒童貧血情況需高度重視,應(yīng)該加強(qiáng)對(duì)該類兒童家長(zhǎng)及兒童本身的營(yíng)養(yǎng)宣教,提高其對(duì)營(yíng)養(yǎng)的關(guān)注度,合理膳食。
[Abstract]:Objective: To investigate the relationship between anemia and physical growth in school-age children. Methods: a multi stage stratified cluster random sampling method was used in the Jiulongpo District of Chongqing, and 6260 school-age children in 5 full-time public primary schools were measured and the blood biochemical indexes were examined.
