【摘要】：目的系統(tǒng)評(píng)價(jià)中國(guó)兒童及青少年幽門(mén)螺桿菌(H.pylori)感染的流行病學(xué)特征。方法檢索Pub Med、EMBASE、Cochrane圖書(shū)館、中國(guó)知網(wǎng)、萬(wàn)方數(shù)據(jù)庫(kù),檢索起止時(shí)間為建庫(kù)至2016年9月。2名研究者根據(jù)納入與排除標(biāo)準(zhǔn)篩選文獻(xiàn),采用AHRQ量表評(píng)估文獻(xiàn)質(zhì)量,使用Stata 12.0軟件進(jìn)行meta分析。結(jié)果 7篇文獻(xiàn)納入meta分析,涉及16 950名兒童及青少年。中國(guó)自然人群兒童及青少年H.pylori總感染率為29%(95%CI:0.18~0.40);兒童及青少年H.pylori感染呈顯著的地域分布性差異,在胃癌低發(fā)區(qū),中國(guó)自然人群兒童H.pylori總感染率為20%(95%CI:0.13~0.28),在胃癌高發(fā)區(qū)H.pylori總感染率高達(dá)55%(95%CI:0.21~0.89);男性與女性H.pylori感染率無(wú)顯著差異;≤1歲、~5歲、~10歲、~15歲、~18歲兒童及青少年的H.pylori感染率分別為17.8%、8%、19%、26%、45%,隨年齡增長(zhǎng)H.pylori感染率有上升趨勢(shì);農(nóng)村兒童及青少年H.pylori感染風(fēng)險(xiǎn)是城市的1.25倍;有消化道癥狀的農(nóng)村兒童及青少年H.pylori感染風(fēng)險(xiǎn)比無(wú)消化道癥狀者高2.2倍。結(jié)論中國(guó)自然人群兒童及青少年H.pylori總感染率為29%,呈現(xiàn)顯著的地域分布性差異,胃癌高發(fā)區(qū)H.pylori感染率是胃癌低發(fā)區(qū)的2.8倍。仍需后續(xù)高質(zhì)量、大樣本量流行病學(xué)調(diào)查,以進(jìn)一步明確中國(guó)兒童及青少年H.pylori感染情況。
[Abstract]:Objective to evaluate the epidemiological characteristics of H. pylori infection in Chinese children and adolescents.Methods the Pub Medbin EMBASE Cochrane Library, China knowledge Network and Wanfang database were searched. The literature quality was evaluated by AHRQ scale according to the inclusion and exclusion criteria from the beginning to the end of the database construction until September 2016.Meta analysis was carried out with Stata 12.0 software.Results Seven articles were included in meta analysis, involving 16 950 children and adolescents.The total infection rate of H.pylori in children and adolescents of natural population in China was 2995 CI: 0.180.400.The distribution of H.pylori infection in children and adolescents was significant difference in the low incidence area of gastric cancer.The total infection rate of H.pylori in Chinese natural population was 20 / 95. The total infection rate of H.pylori was as high as 55 ~ 95CI: 0.21 ~ 0.89 in the area with high incidence of gastric cancer. There was no significant difference between male and female in H.pylori infection rate.The infection rate of H.pylori in children and adolescents aged 15 and 18 years old was 17.80.The infection rate of H.pylori increased with the increase of age, and the risk of H.pylori infection in rural children and adolescents was 1.25 times higher than that in urban areas.The risk of H.pylori infection in rural children and adolescents with digestive tract symptoms was 2.2 times higher than that without gastrointestinal symptoms.Conclusion the total infection rate of H.pylori in children and adolescents of Chinese natural population is 29, showing significant regional distribution difference. The H.pylori infection rate in the high incidence area of gastric cancer is 2.8 times higher than that in the low incidence area of gastric cancer.High quality and large sample epidemiological investigation is still needed to further clarify the H.pylori infection in Chinese children and adolescents.
