過敏性紫癜患兒外周血樹突狀細胞Toll樣受體與共刺激分子表達特性的初步觀察

【摘要】:目的:觀察過敏性紫癜(Henoch—Schonlein Purpura,HSP)患兒外周血中樹突狀細胞(dendritic cells, DC)表面Toll樣受體(TLR)表達狀況,探討TLR表達異常在HSP免疫發(fā)病機制中的作用。 方法:隨機選擇2011年12-2012年7月在青島大學醫(yī)學院附屬醫(yī)院住院治療的HSP患兒20例為研究對象,20例健康體檢兒童為對照組。無菌條件下采集外周靜脈抗凝血,Ficoll密度梯度離心法分離獲得外周血單個核細胞(PBMC),經(jīng)粒-巨噬細胞集落刺激因子(GM-CSF)和白介素4(IL-4)誘導培養(yǎng)獲得DC,采用流式細胞儀(FACS)檢測DC表面TLR2、TLR3、TLR4、CD83、CD86的表達。 結果:①HSP組DC表面CD83表達率[(52.39±4.20)%]與對照組[(52.44±3.87)%]比較,差異無顯著性意義(t=-0.06,p0.05);而HSP組DC表面CD86表達率[(73.81±4.75)%]明顯高于對照組[(60.47±3.30)%],差異有顯著性統(tǒng)計學意義;(t=2.33,p0.05)。②HSP組DC表面TLR2和TLR3表達率均明顯高于對照組分別為[(40.24±5.95)%VS(17.17±3.34)%,(31.31±2.75)%VS(13.47±2.20)%],差異有顯著性(t=2.39,2.60,p0.05),而TLR4表達率兩組間無顯著性差異[(23.51±5.75)%VS(23.47±2.20)%,t=0.03p0.05]。③HSP組與對照組DC表面TLR2和TLR3表達率與CD86表達率皆呈正相關關系(-=0.536~0.941,P0.05),而TLR4表達率與CD86表達率間無明顯相關性。 結論:過敏性紫癜患兒DC表面TLR2、TLR3和共刺激分子CD86表達增強且呈現(xiàn)正相關關系,提示TLRs和共刺激分子表達異常在HSP發(fā)病機制中起重要作用,可能通過啟動以Th2為優(yōu)勢的異常免疫應答參與HSP的發(fā)病。
[Abstract]:Objective: to investigate the expression of Toll like receptor (TLR) on dendritic cells (DCs) in children with Henoch-Schonlein Purpura HSPs, and to explore the role of abnormal expression of TLR in the pathogenesis of HSP.Methods: twenty children with HSP who were hospitalized in the affiliated Hospital of Qingdao University Medical College from 12 to 2012 were randomly selected as the control group.Peripheral blood mononuclear cells (PBMCs) were isolated from peripheral blood mononuclear cells by Ficoll density gradient centrifugation under aseptic conditions, and induced by granulocyte-macrophage colony stimulating factor (GM-CSF) and interleukin-4IL-4 (IL-4).The expression of TLR2, TLR3, TLR4, CD83 and CD86 on DC was measured.Results the expression rate of CD83 on DC was 52.39 鹵4.20% in HSPgroup and 52.44 鹵3.87% in control group.The expression rate of CD86 on DC surface in HSP group [73.81 鹵4.75%] was significantly higher than that in control group [60.47 鹵3.30%], and the difference was statistically significant. The TLR2 and TLR3 expression rates on DC surface in HSP group were significantly higher than those in control group [40.24 鹵3.34 鹵3.34 鹵3.34 鹵3.34 鹵3.34].There was no significant difference in the expression of TLR4 between the two groups [(23.51 鹵5.75)%VS(23.47 鹵2.20) p0.05] .3There was a positive correlation between the expression rate of TLR2 and TLR3 on DC surface and the expression rate of CD86 in the HSPgroup and the control group, but there was no significant correlation between the TLR4 expression rate and the CD86 expression rate.Conclusion: the expression of TLR2TLR3 and costimulatory molecule CD86 on DC surface of children with Henoch-Schonlein purpura is increased and has a positive correlation, suggesting that the abnormal expression of TLRs and costimulatory molecules may play an important role in the pathogenesis of HSP.It may be involved in the pathogenesis of HSP by initiating an abnormal immune response dominated by Th2.
